Job Description The Clinical Care Management Supervisor is responsible for the daily supervision and oversight of clinical team members on the ASAM team. The Clinical Care Management Supervisor is responsible for ensuring that services are authorized according to medical necessity criteria and in accordance with the CBH mission and contractual obligations. Job Description CLINICAL MANAGER PHILADELPHIA, PA CHRISTINE GIBSON PEER SUPPORT HOUSE POSITION SUMMARY The Clinical Manager is responsible for the overall supervision of clinical services and individual supervision at the Peer Support House, which provides on-going crisis residential services to individuals who are involved with the criminal justice system and who without additional support are at risk of experiencing a mental health crisis following release from incarceration or forensic hospitalization. The Clinical Manager will provide direct supervision of the Site Supervisor, Office Manager, and Nurse. Job Description Intensive Behavioral Health Services (IBHS) Supervisor will provide supervision to all workforce members that provide individual services, which includes in-person, on-site, and case reviews to ensure a high quality of care. The IBHS Supervisor will also oversee case documentation and collaborate care both internally and externally to ensure continuity. The Acute Care Manager, Complex Care (RN) is responsible for achieving positive patient outcomes, managing quality of care across the continuum of care with efficient allocation of resources. This role will first and foremost serve as an advocate for our patients and families as they navigate through external providers and healthcare systems. The Acute Care Nurse is an important member of the Complex Care Team and will use all available resources and leverage other members of the healthcare care team to develop effective plans of care and with focus on delivering high levels of longitudinal care coordination. The Acute Care Nurse role also involves establishing relationships with patients’ families and care givers, primary care physicians, hospitalists, specialists, social workers, other case managers and nurses, acute and post-acute facilities, home health care companies, and health plans. The success of this role is determined by management of patients in hospital to ensure patients receive safe and timely discharge to the lowest level of care. This position will focus on health promotion for a senior population providing onsite hospital visits communicating and coordinating care with hospitalist/hospital staff and patient providing appropriate level of care recommendation (inpatient vs observation), using our internal charting system to report daily inpatient updates and working with hospital team on an expeditious discharge, planning to next level of care. The Acute Care Nurse anticipates the need for post-acute and/or long-term care, from day one (1) of hospital stay, providing support to all parties involved. Daily updates in our charting system are required on each patient using the hospitals EMR system and onsite reviews. Acute Care Nurse follows the patient throughout the continuum of care when patient discharges to a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) to provide weekly updates on discharge and ensure that upon discharge patients is connect back to the care of the primary care provider. The Intensive Community Care Manager (ICCM) is a Registered Nurse (RN) who works with our highest complexity patients, their primary care physicians, and other members of the care team that provides hyperfocus case management and field nursing interventions to prevent unnecessary hospital arrivals, keep patients engaged in our intensive primary care model and maximize their healthy time at home. The Intensive Community Managers (ICCMs) will serve as a clinical lead for the Complex Care Team. They will assess, evaluate, and coordinate the team’s efforts to stabilize our highest risk patients, with special areas of focus including safe transitions of care from facilities back to our primary care teams, stabilization of our highest risk ambulatory patients and outreach to patients who are assigned to us but are not engaged in care. This person will perform assessments and design comprehensive plans of care, and drive the actions needed to keep the most complex patients safely at home. This professional will also provide clinical supervision to other team members in delivering the plan of care and in other tasks necessary to meet their needs and engage them in care. As a clinical leader for the team, this person will also be deeply involved in prioritizing team efforts and may also become the direct supervisor for some team members. The Intensive Community Manager works in partnership with the PCPs to draft personalized care plans that address patient’s immediate needs that cause a risk for unnecessary hospital arrivals. This position adheres to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance and policies and procedures. *This role requires being on-site at the Mayfair and Olney locations, with in-home patient visits as needed. The hours are Monday through Friday. CORE JOB DUTIES/RESPONSIBILITIES: • Provides in-house, at facility, and telephonic visits to patients at high-risk for hospital admission and re-admission (as identified by CM Plan) with the main goal of preventing unnecessary hospital arrivals for patients that have consented to the program and after successfully completed full course of program., • Provides home visits to perform field nursing interventions, assess patient, and the development of care plan to identify the goals, barriers, and interventions that will be addressing during the follow up patient visits. Once a patient has completed their episode of care management the register nurse (RN) will review patient chart for discharge and conduct final discharge with patient. Job Description The Crisis Access Link Model Services (CALM) Clinical Supervisor oversees the CALM program by providing leadership to all personnel, in tandem with the Department Leadership, to provide trauma-informed, evidence-based services to children/youth, resource parents and families of origin. The CALM Supervisor will strive to improve outcomes for children/youth entering or moving within the child welfare system by supporting greater permanency, creating critical pathways of connection across systems, and providing supports to prevent crisis/disruption from emerging and creating stabilization. Within twenty-four hours of placement, they will ensure the team meets with the child/youth and resource parent in the home to assess their strengths, protective and risk factors, provide psychoeducation regarding trauma, and connect the child/youth and resource parent to each other and with resources to support placement stability. They will also ensure team will continue to provide on-going, consistent support until warm hand-off is facilitated, as well as work alongside child welfare partners and behavioral health partners collaboratively. A Master’s degree from a regionally accredited college or university in a mental health discipline, such as clinical psychology, mental health counseling, clinical social work, social work or other related discipline. A degree which exposed the student to provision of services through an internship experience in setting with children/youth and on-site supervision preferred. A minimum of one year providing supervision in a child/adolescent services agency is preferred. Job Description The Clinical Care Manager (CCM) Specialist- Children’s Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care. The CCM Specialist will transfer their Wraparound skills to the member and their family, by teaching them to think outside of the box and use natural and community supports. An Outpatient Therapist for PCIT delivers child centered, family-focused PCIT sessions utilizing the evidence-based approach and protocols. Services are delivered in an office setting with full PCIT specifications and an authorization is not required for this service. Treatment is focused on strengthening the interactive relationship between the young child and the parental figure through play based interactions. The therapist utilizes strategies derived from the principles of attachment and behavior modification to help the child and family reach their goals. POSITION RESPONSIBILITIES • The therapist is assigned the responsibility to ensure that the needs of allocated children and families are being properly cared for. Any concerns about a family or child must be immediately communicated to the Clinical Supervisor or then the next higher administrative position whether in or out of office hours, • A therapist must accept any case assigned to him/her. Discussion around allocations will take place and there is room for negotiation, but the Clinical Supervisor makes the final decision. Once a case is assigned, the therapist has to maintain the responsibility for the case up until a point of transfer of responsibility, this point of transfer being the decision of the Clinical Supervisor. This is to ensure continuity of care., • The therapist must carry out all instructions of the Psychiatrist, Clinical Supervisor, and Program Director, precisely and in accord with time-lines. If for some reason, the directives or decisions cannot be carried through, the therapist is responsible for immediately communicating the problem to the Clinical Supervisor and Clinic Director; so that revised decisions can be made. This close communication between the therapist and the psychiatrist and Clinic Director is essential and is a major responsibility for therapists., • If there is an urgent matter or an issue of crisis, the therapist must call one of the emergency mental health facilities for decision making assistance. Has the general responsibility to coordinate, evaluate and deliver life enriching, creative wellness therapy to residents with serious mental illness utilizing recreational, social, cultural, intellectual, emotional, and spiritual programs to address resident’s needs. Will plan and carry out activities in accordance with program guidelines along with the resident's assessment and care plan in collaboration with Life Enrichment Department at West Park Nursing and Rehabilitation Center ESSENTIAL & CORE FUNCTIONS: -Assist residents in working toward treatment goals through creative, life enriching treatment sessions, individual and group projects, and milieu activities with other residents and team members. -Promote the physical, social, and mental well-being of residents by applying experience in activities and knowledge of diagnoses and conditions of residents. -Plan, develop, organize, implement, and evaluate the activity programs of this program. -Follow the care plan in providing daily activities for the residents. -Select group tasks that address plan goals, and observable behavior. -Interview residents and family members to obtain appropriate activity information. -Develop therapeutic relationship with residents to develop realistic treatment plans, with short- and long-term goals. -Create and maintain a calm environment with an atmosphere of warmth, personal interest, and positive emphasis. -Incorporate residents’ cultural and spiritual values and age-related psychosocial developmental needs into program planning. -Arrange physical environment in preparation for activities, including moving/arranging of equipment/supplies to assure that activity areas are ready for scheduled activity while ensuring resident and guest safety. -Use and operate activity equipment and supplies in an efficient safe manner. -Assist in cleaning up after completing activity functions. -Create/provide materials, decorations and set up necessary equipment for activity functions. -Escort residents to and from the activity area. Arrange transportation for field trips when necessary. -Assist in scheduling movies, planning parties, and providing games/activities for residents. -Closely supervise and oversee the residents engaged in scheduled activities. -Observe and document residents’ attendance, mood, behavior, progress, and degree of involvement in activities. -Complete progress notes that are informative and descriptive for the services provided and indicate the resident's response to the service. -Complete MDS requirements and care plan updates as needed. -Encourage residents to participate in recreational activities, developing strategies for motivating residents as needed. -Make routine visits to residents and help with crafts, projects, etc., as necessary. -May arrange for the sale of articles made by residents, i.e., at bazaars, in gift shops, etc. Assist bedridden residents by visiting them, writing letters, running errands, as necessary. -Encourage residents to develop their educational development through reading, etc. -Anticipate and plan for special or seasonal events, including appropriate evening, weekend, and holiday programming. -Develop and maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. -Review complaints and grievances made by the resident and make a written and oral report to the Program Director. -Identify resources for community outings and educate residents about the use of community resources with an emphasis on developing leisure resources and skills that can be integrated into aftercare planning. -Participate in facility surveys (inspections) made by authorized government agencies, as necessary. -Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Program Director as required. -Assist in maintaining an ongoing quality assurance program for the activity department. -Maintain confidentiality of all resident care information. -All other duties as deemed necessary and appropriate, or as may be directed by the Program Director. EDUCATION & EXPERIENCE: Bachelor's degree in behavioral health or related field preferred. Certification as a Therapeutic Recreational Specialist (CTRS) is preferred. Previous working experience in Long Term Care (LTC) mental health, preferably with the frail older adult with serious mental illness population is required. Previous experience as an activities/life enrichment worker preferred. Bilingual capabilities are a plus. Hourly Rate: $22-$25 PROGRAM DESCRIPTION: The Philadelphia Model: Addressing serious mental illness (SMI) in long-term care (LTC) will provide highly structured therapeutic residential behavioral healthcare and treatment for frail adults who are eligible for skilled nursing level of care who also have serious mental illness and who have reached maximum benefit from the mental health resources available elsewhere in the community or hospital. The program philosophy is to create a welcoming, safe, and supportive environment where frail older adults with SMI that may include other complex behavioral health needs and/or prior criminal history – and need long-term skilled nursing services – can receive the care and services needed to address their holistic needs free of stigma. A multidisciplinary team will provide both long-term care and behavioral health services in this integrated program.
