Job Description JOB SUMMARY Background: The Office of Mental Health (OMH) and the Department of Health (DOH) are co-defendants in the Stipulation and Order of Settlement, United States v State of New Your, Civil Action No. 13-0CV-4165 (NGG) O’Toole et al. v. Cuomo et al., Civil Action No. 13-CV-4166 (NGG). This settlement requires the state to assist residents of impacted adult homes identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap-around services are in place after the move. An assessment determines the care planning that the care manager must implement as part of the preparation for transition out of the Adult Home. In collaboration with the Nursing Assessor and assigned Care Coordinators, the Social Work Assessor will work with other team members to streamline the assessment and placement into supported housing for identified residents of impacted Adult Homes. They will be responsible for making recommendations about the type of housing and the types of services—physical and behavioral health—as well as non-medical services like Meals on Wheels, etc. The Social Work Assessor will complete assessments for Queens Adult Home and SIBN Adult Home Supported Housing Programs. The Social Work Assessor will complete the housing application's psychiatric evaluation and psychosocial portion. The Nurse Assessor will have primary responsibility for assessing the residents' physical health needs, such as Managed Long-term Care services and certified Home Health Agency services. Each assessor will interface with the respective behavioral or physical health providers of the residents and review records necessary to make determinations about the types of services that will be needed to promote successful placement in supported apartments.
