Your Core Responsibilities: • Provide respite services to families and individuals in the In-Home Respite Program, offering relief to caregivers and companionship to individuals with disabilities., • Deliver all assigned service hours in the person’s home or in community settings (social activities, appointments, stores, etc.)., • Provide face-to-face support and document only face-to-face time for billing purposes., • Assist with personal care tasks such as toileting, dressing, and eating as needed., • Work independently while maintaining communication with your supervisor regarding schedules, absences, or changes., • Complete all required documentation accurately and submit by assigned deadlines., • Ensure the safety, oversight, and protection of assigned individuals through ongoing risk assessment and monitoring., • Follow all OPWDD and NYS Justice Center reporting requirements., • Adhere to Lifetime Assistance’s mileage, transportation, and service authorization policies., • Attend staff meetings and training as requested. What You Bring: • High school diploma or equivalent preferred., • Must hold a valid NYS Driver’s License (have had your driver's license for 6 months), have a reliable vehicle, and meet agency vehicle operator requirements., • Ability to read and interpret service plans, safety procedures, and related documentation., • Strong written and verbal communication skills to interact with individuals, families, and service providers., • Ability to solve practical problems, think independently, and adapt to different environments., • Physical ability to assist with personal care tasks and lift up to 35 pounds as needed.
