Job Description Position Title: Registered Nurse II (RN II) AND Charge Nurse Department: Nursing Reports To: Director of Nursing POSITION SUMMARY This position is primarily responsible for providing direct patient care services to a diverse patient population through the administration of medication, counseling, and health maintenance. Assesses patient’s health needs and resources, and provides technical nursing care and interventions to designated patient populations. Functions as charge nurse on the units as designated by Director of Nursing (DON) or designee. This position requires providing service to one or more age populations including adolescents and geriatric patients in a manner that demonstrates an understanding of the functional/developmental age of the individual served. In addition, nurses shall have the ability to assess, treat, and appropriately interact and care for individuals with mental health and substance use disorders. This position requires the full understanding and active participation in fulfilling the mission of Tarzana Treatment Centers, Inc. (TTC). The employee is expected to demonstrate behavior consistent with the core values. The employee shall support TTC’s strategic plan, and the goals and direction of the Performance Improvement Plan (PIP). The RN II level is determined at the time of interview and through demonstration of competency in the RN I role. The RN II is the next level of advancement following an RN I and includes all the duties of the RN I. The RN II level includes the ability to perform the duties of charge nurse, safely oversight the nursing staff, and ensure the inpatient units are functioning at optimum levels of care and service. The RN II can make sound nursing and administrative decisions about patient care when administration is not on site. The RN lI has experience in behavioral health nursing care or has successfully demonstrated a sound understanding of nursing care for behavioral health and substance use disorder patients. CATAGORIES OF DUTIES • Understands the nursing role on the inpatient units., • Plans and delivers nursing assessments and interventions, related medication, and psychiatric diagnoses of the patient., • Completes pre-admission triage and determines appropriateness of admission., • Administers medications to patients consistent with medication policies, practices, and standards. Must have necessary computer skills to use electronic medication systems., • Communicates significant changes in patient’s condition in a timely manner., • Documents in patient’s chart nursing goals, interventions, and clinical outcomes. Must be able to chart in both electronic and manual patient records., • Functions as an active member of the treatment care planning team., • Responds to medical emergency situations., • Demonstrates sound nursing judgment and applies nursing standards and ethics when dealing with urgent/emergency patient care and nursing matters or has work experience that indicates same., • Competently uses equipment necessary for the delivery of patient care including but not limited to: glucometer, defibrillator, EKG machine, intermittent suction, bag/mask, oxygen, restraints/seclusions., • Attends trainings as required., • Able to provide wound care as ordered by our providers., • Able to identify and report medical issues observed or determined at admission OR observed or identified by staff and notifies the providers as appropriate., • Able to input nursing notes and take provider orders and transcribe in the electronic medical record or the patient’s records., • Able to provide report to the team regarding patient issues and care needs., • Able to represent the nursing team and the patient in meetings in a professional manner., • Understands the principles of treatment planning., • Understands the principles and theories of nursing care., • Able to identify possible infections and report and document as appropriate; initiate standard precautions if needed., • Administers waived tests (urine pregnancy, blood glucose test, breathalyzer, UA drug screen, HA1c and COVID/Flu) and maintains annual waived testing requirements/training., • Performs other job-related functions as may be assigned by the DON or charge nurse. Charge Nurse Duties: The RN ll is responsible for the above duties and in addition, is responsible for the charge nurse duties listed below. • Successfully passed the probation period at TTC or has been approved to function as a charge nurse by the DON., • Is accessible to all staff while on duty., • Is able to make and demonstrate independent nursing decisions that are in the best interest of the patient, the unit, the staff and TTC., • Acts as a mentor and professional role model to all nursing staff on duty., • Represents the TTC administration when the administration is not on duty or not available and reports all incidents/issues to the DON, the Unit Director, or designees in a timely manner., • Is a resource to all staff., • Able to provide guidance and oversite to the nursing staff., • Able to determine if additional nursing staff is needed or if overtime/double time is required to meet care needs in consultation with the DON or designee., • Can assign nursing team duties to meet the needs of the inpatient units., • Reviews and assures daily nursing duties are completed (completes the charge nurse check list daily)., • Provides report to the DON or designee as needed and includes the reason for overtime and/or double time for any nursing staff on duty in the inpatient units., • Is accountable to ensure nursing care is provided per provider orders., • Is dependable and maintains a good attendance record., • Able to identify possible infections, report and document as appropriate; initiate standard precautions if needed., • Administers waived tests (urine pregnancy test, blood glucose test, UA drug screen, breath alcohol tester, and fentanyl urine screen) and maintains annual waived testing requirements/trainings as required., • Performs other job-related functions as may be assigned by the DON or designee. LICENSES/CERTIFICATIONS ▪ An active California Board of Registered Nursing (BRN) license as a Registered Nurse (RN) ▪ An active CPR certification.
