Job Description LOCATION: S. PROGRAM BACKGROUND: Every child should feel cared for, be safe and be nurtured. The goal of our Enhanced Family Foster Care is to provide services necessary to enable each child a safe, permanent circle of support. Whenever possible, reunification with family is pursued. this program strives to meet the needs of New York City’s ever-changing foster care population, Enhanced Family Foster Care (EFFC) . Our program recognizes that all children in foster care have experienced trauma, that their needs may change over time, and that their strengths must be supported so they can reach their full potential. This position is conveniently located in S. Ozone Park, Queens, close to Mass Transit. POSITION SUMMARY: A Caseworker works with children and youth who reside in a foster home as well as family members and other adults serving as supports to the young person. In addition, we support our foster parents, who can either be relatives or could be individuals who are not related to the children. Caseworkers work as a team with Socio-Therapists to provide supports to the children, the birth families and the foster parents to ensure that every child is making progress towards permanency. REQUIRED QUALIFICATIONS: • Bachelors in social work or related field, • 1 year of related work experience in serving children and/or families, • Knowledge of and/or training in child development stages, • Knowledge of and/or training in family dynamics, • Excellent communication skills (verbal and written), • Proficiency in Microsoft Word and Outlook, • Ability to engage with clients, • A valid driver’s license is strongly preferred, • Understanding and willingness to commit to the agency’s Mission, core values, Sanctuary Commitments and Diversity, Equity, Inclusion and Belonging (DEIB). The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs., • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner., • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients., • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication., • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed., • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics., • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist 1. Graduation from an approved medical school., 2. Completion of approved residency or fellowship in the specialty or sub-specialty and Board eligible or certified or Subboard eligible or certified., 3. Five years experience in field of specialty or subspecialty acceptable to the Medical Board of the Hospital., 4. Licensed to practice medicine in the State of New York. Department Preferences • Possession of a valid license to practice medicine in the State of New York; and,, • Completion of three years of approved residency training in psychiatry. Summary of position The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Responsibilities Include (depending on location): • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction, • Ensure that medication renewals are completed in a timely manner, • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards, • In collaboration with the Mental Health Unit Chief, oversee the orientation of new staff to the facility or program, • Conduct regular and multi-disciplinary case conferences, • Report on an as-needed basis to the CHS Morbidity and Mortality review committee., • Work in collaboration with the Mental Health Unit Chief to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education, • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary, • Lead at least monthly interdisciplinary mental health staff meetings in coordination with the Mental Health Unit Chief, Clinical Supervisor and Senior Psychiatrist (if applicable)., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints, • At least 75% of time devoted to direct clinical care, which may include direct supervision of psychiatric trainees. For smaller facilities or programs with less supervisory need, more direct patient care will be expected, • On-call responsibilities for two weeks at a time, one to times/year, for off-hours issues that require high level clinical intervention 1.Master's degree from an accredited college or university in Healthcare Administration, Health Care Planning, Business Administration, Public Administration or a related discipline; and seven (7) years of progressively responsible experience in health and medical service administration, public administration, personnel and labor relations, finance or appropriate functional discipline with an emphasis on planning, liaison and inter-organizational relationships, or related administrative or managerial functions; or 2.Bachelor’s degree from an accredited college or university in disciplines, as listed in “1” above; and eight(8)years of progressively responsible experience in areas, as listed in “1”; or 3.Satisfactory equivalent combination of education, training and/or experience. However, all candidates must have a minimum of a Bachelor’s degree in disciplines, as listed in “1” above. North Infirmary Command (NIC), West Facility (WF), and the Contagious Disease Unit (CDU) are the jail facilities that house a significant number of incarcerated individuals with advanced age, complex medical problems, disabilities, and/or enhanced nursing needs. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for elderly and medically complex patients with comorbid mental illness., • Directly treat and oversee the psychiatric care of additional patients housed in NIC/WF/CDU, • Direct patients to the appropriate level of mental health care, within the facility or in the hospital as needed., • Close communication with psychiatric and medical providers at Bellevue Hospital to ensure continuity of care as patients transition between the two facilities., • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief and/or Senior Clinical Supervisor, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. Department Preferences: • Excellent diagnostic and treatment skills with particular attention to issues of psychopharmacology, safe administration of medication in a variety of settings, medical complications of mental illness and therapeutic resistance, • Excellent interpersonal communication skills and ability to work collaboratively with other disciplines, including DOC staff, • Strong leadership skills with ability to identify staff needs and provide mentorship and education to suit those needs, • Advanced knowledge of psychopharmacology in elderly and medically complex patients, • Experience working with medically ill psychiatric patients, • Buprenorphine waiver training highly encouraged but not required Qualification Requirements: Education • M.D. or D.O. degree with active NY State license in good standing, • Completion of a 4-year psychiatric residency training program, • At least two years of post-residency experience working with individuals with mental illness in a criminal justice setting (jail, prison, probation and/or parole) or public health community setting, • At least one year of post-residency experience in a supervisory role in psychiatry, • At least one year of post-residency experience working with individuals with serious mental illness, • An equivalent and satisfactory combination of education and experience may be considered.

Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. Principal Responsibilities • Assign advocates to all residents upon arrival to NSP. >Facilitate no less than 5 therapeutic group meetings consistently, • Assist in the development of a comprehensive program schedule/activity plans w/site director., • Assist the Unit Manager in facilitation of weekly team meetings., • Coordinate all staff schedules two weeks in advance with final approval from site director., • Secure staff for all appointments, i.e. medical, court, probation, outside therapy, etc., • Meet with each shift bi-monthly for group supervision around strengths/challenges. Submit documentation of meetings within 48 hours to site director/NSP residential director., • Meet w/ NSP residential director bi-monthly; attend all meetings/trainings as assigned., • Ensure all shifts maintain a minimum of a 1 to 4. In the case the unit (on any and all shifts) is short staffed the group leader is required to fill in., • Support SCM trainings upon certification., • Collaborate with all services providers both internally/externally to meet needs of youth., • Support residents in working on treatment activities and petitioning for phase in a timely fashion., • Hold all shifts accountable (morning, evening, over-night Monday through Sunday) to follow the schedule and implement all activities as required., • Attend no less than 3 treatment planning conferences per resident per month (1stmtg, 3rd month meeting and final discharge meeting)., • Organize resident drop off and pick up, assign staff as needed and ensure all residents are back in the facility no later than 5pm on Sunday., • Attend all special events e.g., family day, St. John’s day, community service projects, trips, etc., • Additionally, beyond the aforementioned statements, perform any and all tasks as assigned by program leadership. Required Qualifications and Experience • Associate's Degree in Human Services or related field preferred, • Must have a minimum 1year experience working with vulnerable youth, • Individual must be well versed in the MYSI approach/standards, experience facilitating group meetings in an NSP setting utilizing the MYSI approach, knowledge of NSP programming., • A valid driver’s license and strong communication skills with the ability to manage appointments (i.e., medical, court, probation, therapy, etc.) for all of the residents. Job Description Summary Job Description: Shift Supervisor is accountable for the physical/emotional safety of the environment while on shift. Additionally, the individual is responsible for leading the staff/resident teams utilizing the Tuckman model of supervision along with the MYSI standards (Eyes on, Ears on, Heart on)/ therapeutic approach in the Department of Education setting. The individual is accountable for staff engaging youth in any and/all education components throughout the school day and during after-school in terms of homework, tutoring and after-school programming. Moreover, the Shift Supervisor must ensure that the behavior management specialist team is positioned optimally to support youth in each educational component. Furthermore, the Shift Supervisor is responsible for the overall safety and movement of the resident/staff team to and from school. This includes timely arrival to school daily, and monitoring speed of agency vehicle/driving route (if applicable). This position offers a full-time, 40 hour-per-week, in person work schedule and is based in Staten Island. From time to time, this position requires work outside of normally scheduled working hours. About St. John’s Founded in 1826 as a home for orphaned children, St. John’s has evolved into a nonprofit social services agency providing residential care to New York City youth involved in the child welfare and juvenile justice systems. We offer holistic, trauma-informed support to young people up to age 21. Our main campus in Rockaway Park, Queens, houses a Residential Treatment Center, Close to Home Non-Secure Placement programs, administrative offices, clinical services, and an on-site NYC Department of Education school. We also operate a Non-Secure Detention residence in Richmond Hill and Limited Secure Placement programs in Brooklyn and Staten Island. Principal Responsibilities • Demonstrates fidelity to the agency's service values and organizational imperatives., • Responsible for the full engagement of all residents in each educational component on a daily basis., • Ensures pat frisk are conducted twice daily to minimize the introduction of contraband into unit., • Ensure all tasks are completed prior to team’s departure, this includes but not limited to incident reports, daily log and log book, etc., • Must debrief the current/ incoming shift members each and every day without fail., • Responsible for the timely arrival of residents to and from school on a daily basis, including but not limited to driving routes/speed of vehicle (if applicable)., • Maintain a classroom environment that is conducive to learning throughout the school day., • Divides youth into small clusters to be supported by all school youth counselors throughout the day during classtime. All staff are required to complete daily index cards on youth assigned M-Fri to be collected and submitted to the office manager/ designated POC for documentation., • Responsible for supporting residents in completion of all daily school/other assignments/ tasks and support residents in creating a daily routine of studying both during and after-school., • Responsible for strategic positioning and engaged supervision of staff and residents in each classroom, during school hours., • Maintain all relevant certifications including but not limited to a clean/valid driver’s license, SCM certification, etc., • Additionally, beyond the aforementioned statements, perform any and all tasks as assigned by program leadership. Required Qualifications and Experience • Associate’s degree in human services or related field and a minimum of 1 year relevant experience working with vulnerable youth and families preferred -OR- a high school diploma and 2 years of relevant experience working with vulnerable youth, • Valid driver’s license, • Individual must be well versed in the MYSI approach/standards, TCI/ SCM/Other training specific to program, • Experience managing behavior utilizing the MYSI or other therapeutic approach, strong knowledge of programming and program protocol., • Strong communication skills, • Mature, responsible and effective communicator with proven ability to manage both upward and downward, • Attention to detail, • High level of integrity and independent thinker and doer, • Assumes responsibility for staff and residents during shift, • Leads by example Travel Requirements This position offers a full-time, in person schedule and is based in Staten Island and does require travel to other locations when needed.
