Job Description Job Summary: The Case Manager is responsible for supporting the Program Social Workers in the provision and coordination of casework services for youth of the Runaway Homeless Youth program Job Responsibilities: • Provide casework services to residents of the shelter including referrals for entitlements, housing, and services, accompanying residents on appointments and fostering/facilitating relationships with other services providers to meet the needs of the residents of the program., • Maintain up-to-date case record documentation and reports including progress notes court reports, and all documentation required by city and state governing bodies., • Enter case information into DYCD’s PTS and other demographic Systems. Provide timely communication to DYCD, • Developing, updating, following up on service/treatment plans. Participate in staff meetings, multi-disciplinary meetings, and planning meetings, • Ensure the safety of all young adults on their caseload., • Ensure that all residents are in possession of necessary legal documents, i.e., birth certificate, Social security card, NY State ID medical insurance., • Foster/facilitate relationships with other service providers to meet the needs of the residents of the program., • Assist residents with Independent Living preparation, employment, and educational services which may also include travel training., • Facilitate Independent Living Skills Group workshops for residents., • Develop discharge plans and support networks for residents in the program., • Provide individual counseling in the facility., • Maintain daily communication with Residential Counselors and Program Administrator., • Other duties as assigned by supervisor Qualifications • Bachelor’s degree in human service-related field preferred., • Good writing skills., • Strong Computer knowledge required. No Work Environment: This position is based at one of our Drop-in Centers or Transitional Independent living facilities for Runaway/Homeless youth between the ages 14-24 Position Type/Expected Hours of Work: This Position is a 40hr work week. Travel (if required): Occasionally Physical Requirements: Must be able to pass Therapeutic Crisis Intervention (TCI), including the physical portion. Other Duties: • Act as a role model - guiding clients and facilitating appropriate behavior about daily living skills, self-care, personal interaction, social relationships, and constructive time management., • Maintain and promote a cooperative, harmonious, teamwork environment, interacting with clients and staff in a professional and courteous manner.
