Job Posting: Client Support Coordinator Location: Rochester, NY Department: Care Coordination- Aging Services Employment Type: Full-Time Schedule: Monday- Friday, 40 hours Salary: $21.00/hr. General Description Under general supervision, coordinates in-home support for program participants such as transportation, ancillary purchases, and home repair projects. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma-informed care environment. Essential Duties and Responsibilities • Each staff person might be requested to be back up to a different program and to be flexible to fill in work time to give our participants the best support possible., • Coordinates special transportation requests with the STAR team to ensure appointments are confirmed and timely. Involves taking accurate requests, confirming, finding providers, and strong communication skills. May transport clients to necessary appointments providing advocacy and support as back-up., • Coordinates with other staff the approval, arrangements, ordering, tracking and delivery of needed Ancillary purchases – such as equipment, home repairs, appliances, home modifications, personal items. Involves home visits, communicating with providers/MCOFA/finance/team members, and arranging for various service estimates. May involve home visits and evaluating appropriateness of the purchase., • May provide client assistance with grocery shopping, prescription pick-up, banking, friendly visiting, essential errands and light chores. May provide housekeeping services to clients such as making and changing beds; dusting, vacuuming, light cleaning of kitchen, bedroom, bathroom; dishwashing; preparing light meals; making list of needed cleaning items or grocery items; laundering, ironing; grocery shopping., • May provide back up support to Intake Processor., • Works with participants to use technology to improve their quality of life. Involves home visits and knowledge about internet, smart phone use, online services (banking, telehealth, grocery ordering etc.) and the ability to help them to install or use the services., • Must be able to lift and carry at least 20 pounds and use stairs., • May provide supervision services to clients (respite for caregivers). Monitor client’s safety., • Schedule appointments and necessary confirmation/follow-up for assigned clients as needed., • Keep clients appraised of available community resources and facilitate referrals., • Maintains accurate activity records., • Follows established procedures and policies for programs, such as receipt of cash/checks from clients, • Keeps accurate mileage and timesheet records., • Maintains up to date client service schedule on agency system (such as Outlook)., • Maintain client service logs and accurately records Trips and Contacts and Ancillary Purchases. Submits unit reports to supervisor monthly/weekly depending on the program., • Report problems, incidents to supervisor in a timely manner., • Assist program management staff in completion of required clerical and/or administrative work as needed. (i.e. intakes/outreach), • Assists the Homeworks’ Volunteer with administration of that service; recruitment, background checks, enrollment, payments, scheduling jobs, follow up visits with clients and tracking., • Ensure personal vehicle up-keep and repair to include, but not limited to, inspections and oil changes., • Complete annual or triennial defensive driving course., • Participates in relevant Agency meetings and/or trainings, • Actively pursues development of professional competencies related to job performance through reading, supervision, in-service training and attendance at conferences and workshops., • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse, • Other duties as assigned. Requirements: Qualifications Education: High School Diploma required. Credentials: NYS Driver License Experience: Proven experience in at least one of the following areas. Working with older adults, transporting people, home maintenance and repair, customer services, or providing in-home supports (aide). Must be able to use a computer for scheduling, communication and tracking purposes.
