Overview Provides direct clinical services including assessment and treatment planning, supportive counseling and supportive therapy, advocacy and psychoeducation, and linkage to community agencies and services, as required. Ensures access and linkage to the full array of necessary physical and behavioral health services and other community-based services to address social determinants of health. Works under general supervision. The WeCARE Wellness Program, funded by the Human Resources Administration, provides coordinated care for clients with medical or behavioral health conditions who are temporarily unable to work. After a clinical assessment, eligible clients are assigned to a care manager who maintains a caseload of 70 clients and provides monthly follow-ups. Care managers connect clients to essential treatment providers and community resources to help stabilize their condition. The program includes support from wellness health coaches, who conduct outreach, facilitate workshops, and assist the care management team, as well as an operations team that ensures smooth program delivery. We also have Qualified Health professionals, such as licensed social workers, who complete clinical assessments and manage clients who need enhanced case management. Education: Associate's Degree in Nursing from an approved program required Bachelor's Degree in Nursing from an approved program preferred Work Experience: Minimum of two years in a clinical background as a Registered Nurse required Home care experience in a licensed home care service agency or CHHA settings preferred Pay Range USD $77,200.00 - USD $96,500.00 /Yr. Overview Provides and coordinates post-orientation field support and skills development activities to support the onboarding of newly-hired RNs and LPNs during their transition to VNS Health; provides similar support to incumbent staff, as needed. Works under general supervision. Licenses and Certifications: A license and current registration to practice as a Registered Professional Nurse in New York State required and Valid driver's license required and OASIS certification preferred For Hospice Only: Wound Care Certification required or willingness to obtain within one (1) year of hire required Certification in Hospice and Palliative Care Nursing required Education: Bachelor's Degree in Nursing OR Associate’s degree in Nursing required Work Experience: Four years’ clinical home care experience required and Minimum of two years full-time clinical home care experience required and Minimum of one year of experience as a field staff nurse in VNS Health Home Care, which includes ability to manage/document cases with a minimum of supervision, identify/assess/formulate/implement action plan to address problem and evaluate outcome preferred and Joint visits with other nursing staff to demonstrate new techniques preferred Hospice Only: four years' clinical hospice experience or clinical home care experience Hospice Only: Minimum of two years' full-time clinical hospice or home care experience Pay Range USD $119,258.00 - USD $145,458.00 /Yr.
