POSITION SUMMARY Meet with patients and families to explain the scope of hospice services, coordinate admission of patients and ongoing plan of care. ESSENTIAL FUNCTIONS • Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME., • Provide emotional support to patients and families while instructing them in activities of daily living., • Complete daily paperwork and charting, thoroughly and accurately according to Policy., • Registered Nurse in the State of Texas, • Hold a valid driver's license and have a dependable means of transportation., • 2 years of nursing experience required, • 2+ years of experience in hospice strongly preferred To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
