Job Description Position Summary: CTT is mandated to provide service to those individuals within the mental health system who present the greatest degree of severity of symptoms as evidenced by their multiple physical, psychiatric and legal conditions; extensive use of services and lack of follow through with treatment. These individuals measure at the highest level of medical necessity as defined by the state Adult Environmental Matrix. The ACT Teams are specialized teams that provide multidisciplinary clinical intervention, treatment, and assessment of individuals who may be young adults, heavy users of services, or actively involved in the criminal justice system and may also have co-occurring behavioral health diagnoses. ACT team staff provides case management supports consistent with the Community Support Program of Philadelphia. The Extended Care Vocational Specialist provides a comprehensive assessment process with the intent to offer recommendations and renders therapeutic and vocational services as required to address issues that might present a barrier to flourishing in the community. The Extended Care Vocational Specialist is also responsible for ensuring that the mission, goals and philosophy of the Community Treatment Teams (CTT) are operationalized within the team. The incumbent encourages strengths-oriented treatment/service planning that addresses the individual's principal needs, ensures continuity of care and enhancement of individual's understanding, acquisition and maintenance of independent living skills in the areas of: (a) compliance with behavioral health treatment; (b) interpersonal skills and social support/network building; (c) acceptance of personal responsibility in recovery and (d) accountability in fulfilling their legal obligations in order to remain in the community. Duties and Responsibilities: • Provides comprehensive vocational assessments for assigned individuals; including writing comprehensive reports, • Participates in daily multidisciplinary team meetings to review progress on cases in order to ensure communication and a comprehensive approach to treatment, including collaborating with external teams to ensure coordination between systems; participates in group clinical supervision with the CTT Clinical Coordinator for skills enhancement, • Provides input in case reviews to court and other authorities, as deemed necessary and appropriate, • Provides 80% of available time in individual and/or group vocational activities for individuals on the team, with the goal of competitive employment, • Views individuals from a strengths-approach in preparation of treatment plans, with a focus on recovery principles, • Monitors service delivery to individuals to ensure appropriate level of service given, defined by Environmental Matrix score, • Establishes networks and liaises with community service providers to resolve problems and overcome barriers to service; including attending linkage and treatment team meetings, as needed; provides advocacy mechanism to ensure that individual needs are met, • Maintains and updates community resource file on vocational opportunities and contact people and develops employment prospects in conjunction with CTT Vocational Resource Specialist Completes all required reports and other documentation in a timely manner Maintains compliance with all applicable behavioral health treatment standards as set by state and county funding authorities and ensures that agency standards, policy and procedures are adhered to by team members, • Participates in Clinical Care, Open Forum and other required agency and external meetings; attends in-service and other training in order to meet mandated training hours Plans and assists individuals to increase community tenure, enhance quality of life and attain highest level of independent functioning, • Provides after hours work including: a) participation in weekly on call rotation schedule to permit 24 hour/7 day a week access to service; b) and participation in evening and weekend shift work schedules required for delivery of services to CTT's caseload Must have excellent verbal and written communication skills; ability to work well with people; have good, creative problem solving skills; excellent clinical skills including assessment; individual and group therapy, knowledge of behavioral health and criminal justice systems; community resources, computers skills, be able to work independently as well as part of a team; flexible and adaptive in handling changing priorities in a fast paced work environment; able to work extended hours as may be required to complete projects. Education and Experience: Bachelor's degree preferably a degree in Rehabilitation Counseling or a Master's degree in (including, but not limited to) social work, psychology, rehabilitation, counseling, activities therapy or criminal justice with at least two (2) years of experience in clinical assessment and therapeutic work; one of which must have been in vocational service delivery. Expertise preferred in various therapeutic treatment modalities and one (1) or more of the following areas: criminal justice, substance abuse or geriatric issues. Physical Demands: Ability to sit for at least one (1) hour at a time and to use a computer and telephone. Able to climb stairs at participant's residences; bend, stoop, push and pull file drawers, and lift up to 201bs when handling supplies, etc. Able to work in the community for a significant part of the day. Essential Functions: Must have a valid driver's license, auto insurance and must have use of a vehicle for work and on call; ability to work in a team environment and share office space; ability to work evenings and weekends in meeting responsibilities. The Blended Enhanced (BE) Team works with individuals who measure within the mid to at the high level of medical necessity as defined by the state Adult Environmental Matrix. The BE Case Manager provides team case management supports to all individuals of his/her assigned team and other internal BE teams as the need arises; as part of the Community Support Program of Philadelphia. A major goal of the CTT is to assist the individual in developing skills required to enhance his/her comfort and ability to function as a productive member of the community. The BE Case Manager will ensure that the mission, goals and philosophy of the CTT are operationalized within the team. The BE Case Manager works with individuals in their community environment assisting them in understanding, acquiring and maintaining independent living skills in the areas of: (a) their daily living situation; (b) interpersonal skills and social support/network building; (c) leisure and recreational support/skill development; (d) maintenance and enhancement of physical and mental needs; (e) obtainment of benefit entitlements and the skills to manage same; (f) vocational and educational needs. Duties and Responsibilities: • Assesses individual’s strengths, needs and wants, utilizing instruments to operationalize data on the consumer’s behalf. This includes completion of Strengths Assessment and client-centered Unified Personal Plan for each assigned individual, involving all treatment team principals for comprehensiveness., • Works collaboratively with multidisciplinary team internally on daily basis and external as needed to ensure coordination between systems; provides linkage with primary care physician to ensure integration of medical and psychiatric service needs; maintains linkage with CBH to minimize person’s reliance on acute services., • Periodically completes Environmental Matrix to determine level of care need and delivers services according to individual’s need, ensuring appropriate frequency of contact., • Monitors individual’s progress toward attainment of identified goals through monthly Linkage Meetings to review same; goals should be short term, measurable and obtainable. Plans should be updated according to regulatory standards and all related activities documented on appropriate agency forms., • Liaises, communicates with and represents individual via telephone contacts and face-to-face meetings. This includes regular contact with individual and community service providers in order to respond to individual’s changing needs, assist in problem resolution and provide advocacy mechanism to ensure that needs are met., • Participates in in-patient treatment team and discharge planning meetings; monitors individual’s treatment and progress during hospitalizations., • Maintains and updates community resource file around geriatric and generic resources to assist individual in accessing same and providing necessary training around use in order to facilitate individual’s ability to use resources independently., • Accompanies individual to appointments (financial entitlements, medical, dental, court, probation, etc.) to provide support and assistance., • Provides training to individual on use of public transportation, job seeking skills, identification and use of social and recreational resources, etc., • Acts as team specialist in one of the following areas: housing, benefits, D&A or vocational; including attending periodic specialist meetings as required., • Participates in daily team meetings and Clinical Care Meetings to problem solve around persons needing extensive services for specific times; attends Open Forum and other required agency meetings., • Attends in-service and other training's in order to meet mandated training hours., • Completes required documentation in timely manner., • Assists individual to increase community tenure, enhance quality of life and attain highest level of independent functioning. Education and Experience: Bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling or education with two (2) years’ related experience preferred; Bachelor’s degree in nursing with three (3) years’ mental health direct care experience; be a Registered Nurse (RN); High School Diploma and five (5) years of mental health direct care experience in human services with employment as an ICM staff person prior to April 1, 1989; Bachelor’s degree in other disciplines with 12 social science credits; or High School Diploma plus 12 social science credits and five (5) years’ experience in human service with two (2) years as an ICM technician or equivalent. Valid Firearms, Carry Permit and valid Armed Guard license (Armed-SORA) Preferred. Ready to make a difference as a canine handler. Previous canine experience is not required, paid training provided. Hourly Pay Rate: $30.10. Canine Stipend provided. Required for this position minimum of two years one of these qualifications' military, law enforcement or corrections.
