Job Description BAYADA Home Health Care is in search of a Director of Home Health Clinical Practice, who will report to / works directly with the Home Health leadership team to ensure proficiency and continuous performance improvement in clinical operations, care delivery, revenue integrity, and regulatory requirements. This leader holds accountability for delivering industry leading standards of care while continuously evaluating and evolving the care delivery model to maximize efficiency and resource utilization. The Director of Clinical Practice will collaborate with practice leadership, as well as all clinical and support roles that affect clinical practice, including, but not limited to, the Home Health Directors of Clinical Operations (DCO), Directors of Clinical Education, Clinical and Rehab Managers, and informatics leaders. This role also serves as a conduit between Home Health clinical teams and related Enterprise teams, including Policy, Regulatory, Quality Assurance and Compliance, to maintain the highest standards of care and ensure appropriate payment for those services. This leader is also responsible for supporting the implementation of new care models, leveraging existing and emerging technology while adapting to new billing and payment requirements. While this is a remote role, candidates should have ability to trave as needed (averaging 15% of the time). MINIMUM QUALIFICATIONS: 1. Holds a current license in good standing as a Registered Nurse in the state of practice., 4. Minimum of five (5) years of home health care clinical leadership or equivalent healthcare experience., 5. Completed Continuing Education Requirements (CEUs) as required by regulation., 6. Demonstrated expertise in high quality clinical service delivery and multidisciplinary team management., 7. Knowledge of health care environment as it relates to acute and post-acute delivery systems., 8. Understanding of key quality metrics and best practices for reporting / metric development., 9. Financial acumen and demonstrated understanding of billing models and budgeting processes., 10. Superior communication, organization, prioritization, analytical, and critical thinking skills., 11. Demonstrated ability to take initiative and be a self-starter., 12. Strategic and innovative problem solver who sees the big picture and works with a positive attitude., 13. Track record of successfully working as part of a collaborative strategic team., 14. Track record of developing and implementing successful strategic plans, including project management and change management., 15. Acceptable pre-employment assessment results., 16. Ability to travel, up to 15%, for meetings, trainings, etc. PRIMARY RESPONSIBILITIES: 1. Demonstrate and communicate the core values of BAYADA and The BAYADA Way., 2. Develop working knowledge of BAYADA's mission, services, people, organization, policies and procedures., 3. Promote collaboration and communication between clinical stakeholders within the home health practice and other departments within the organization, fostering an integrated approach to patient care., 4. Support the practice leadership team in setting strategy, determining priorities/goals, and developing strategic plans to achieve those goals., 5. Collaborate with and support the Division Director of Clinical Strategy, Innovation and Care Delivery to influence clinical strategic decision-making considering the shifting industry, payor, and regulatory dynamics., 6. Collaborate with the BAYADA Enterprise clinical teams to ensure alignment of Practice and Enterprise clinical priorities (e.g., policy review, infection prevention, quality assurance and performance improvement, audit compliance)., 7. Act as a member of the Practice Enablement team, maintaining awareness and alignment with all practice projects and initiatives., 8. Contribute content and subject matter expertise to clinical education for all clinical roles in the office and field., 9. Coordinate and facilitate clinical forums, including DCO, Clinical Manager/Rehabilitation Manager, and other councils and workgroups, to ensure input and standardization of clinical best practices., 10. Ensure compliance with organizational policies, regulatory standards, and accreditation requirements. Act as a lead source of support for survey needs (CHAP, State/Federal) and performance improvement planning., 11. Review and support clinical documentation initiatives to ensure quality and compliance., 12. Lead initiatives to improve the care delivery system to reduce variation and improve outcomes, including new clinical programs, clinical guidelines, and evidence-based interventions to enhance patient care and stay ahead of industry trends., 13. Evaluate operational performance to determine if clinical objectives and tactics are achieving desired results; adapt plan as needed to achieve desired results., 14. Ensure continuous improvement in care delivery methods that prioritize the achievement of nationally recognized performance outcomes, such as Star Ratings, Client Satisfaction, and value-based payment programs., 15. Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal for Office Staff., 16. Perform related duties, or as required or requested by supervisor.
