Job Description Charge Nurse LPN Sans Souci Rehabilitation and Nursing Center is seeking a Charge Nurse Charge Nurse LPN to join our team. Charge Nurse Charge Nurse LPN Responsibilities: * Patient Care Management: * Assess, plan, implement, and evaluate patient care plans in collaboration with physicians and other healthcare professionals. * Monitor and manage patient progress, addressing changes in condition and implementing appropriate interventions. * Ensure that all patients receive individualized care based on their needs and preferences. * Staff Supervision and Coordination: * Supervise and support nursing staff, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs). * Assign duties and manage work schedules to ensure adequate staffing and coverage. * Provide guidance, training, and mentorship to nursing staff, fostering a collaborative and positive work environment. * Quality Assurance: * Ensure compliance with facility policies, procedures, and regulatory requirements. * Conduct regular audits and assessments to maintain high standards of care and identify areas for improvement. * Address and resolve patient and family concerns, ensuring satisfaction and effective communication. * Documentation and Reporting: * Maintain accurate and timely documentation of patient care, including assessments, interventions, and outcomes. * Prepare and submit reports on patient care, staff performance, and any incidents or concerns to facility management. * Emergency Response: * Respond to medical emergencies promptly and efficiently, coordinating with emergency response teams as necessary. * Provide leadership during crisis situations, ensuring the safety and well-being of patients and staff. * Interdisciplinary Collaboration: * Work closely with physicians, therapists, social workers, and other healthcare professionals to develop and implement comprehensive care plans. * Participate in interdisciplinary meetings and contribute to care planning and decision-making processes. Charge Nurse Charge Nurse LPN Qualifications: * Education: Associate’s or Bachelor’s Degree in Nursing from an accredited institution. * Licensure: Valid Registered Nurse (LPN/RN) license. * Experience: Minimum of 2 years of nursing experience, with at least 2 years in a skilled nursing or long-term care setting preferred. Previous supervisory experience is a plus. * Skills: Strong leadership, organizational, and communication skills. Ability to make sound clinical decisions and manage multiple priorities effectively. * Certifications: Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
