Job DescriptionDescription: POSITION TITLE: Hospice House Manager REPORTS TO: Hospice House Director FLSA STATUS: Exempt SCHEDULE: Full Time Essential Functions Manages, leads, and coordinates skilled, quality patient care and services for up to 25 Hospice House patients by an interdisciplinary team. Supervisory Responsibilities Direct Hospice House Nurse Manager, RNs, LPNs, CNAs, Social Workers, Chaplain Indirect Housekeeping, maintenance staff and dietary staff Challenges in Position Flexible work schedule, including evenings and weekends. Will encounter patients, families, and staff in high emotional states. Coordinate several tasks at one time with frequent interruptions. Deal with sensitive and confidential issues concerning staff, patients, and families Decision Making Latitude Individual Decision Making Parameters Immediately address staff performance concerns, manage routine patient/family issues, day-to-day management of the Hospice House. In Collaboration with Management Disciplinary actions that require more than verbal or written counseling; unusual or complex patient or family issues. A minimum of 5 years experience as RN, minimum of 2 years hospice RN experience, with 1 year of supervisory experience preferred. Certificates, Licenses, Registrations, Vaccines • Unrestricted Registered Nurse GA license or multi-state RN license, • Hospice and Palliative Care CHPN certification preferred or o Attain within 1 year of employment required, • CPR Certification required, • Valid state Driver’s License, • Car Insurance per Columbus Hospice policy Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to define problems, collect data, establish facts, and draw valid conclusions. Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from families, patients, peers, groups of managers, clients, customers, and the community. Ability to communicate well orally and in writing, with strong interpersonal skills to provide effective leadership to interdisciplinary team members. Position Requirements (not all-inclusive) Provide individualized patient care to include the following: • Daily management of the Hospice House staff and operations, to include communication with staff regarding patient care and documentation, and overall management of staff., • Coordination of patient care to hospice patients in the various levels of care, and monitoring for eligibility and documentation of those levels of care., • Knowledgeable of Medicare Conditions of Participation for Hospice & Inpatient Hospice, Medicaid Regulations, Joint Commission standards, Georgia State Hospice regulations, Alabama State Hospice regulations (including Medicaid), and Columbus Hospice Policies & Procedures., • Understanding of and ability to guide others in the provision of aggressive symptom management, as well as comfort care., • Able to work with diverse individuals and personalities, to include patients, families, physicians, other community health care providers, and Columbus Hospice staff., • Familiarity with Columbus Hospice House physical facility and equipment, including oxygen system, emergency power generator, patient bathing system, electrical systems (call lights, etc.) and other facility specifics., • Responsibility for staffing and other personnel issues for Hospice House staff. Project staffing needs based on patient acuity and census. Create timely and comprehensive staff schedules to meet patient needs., • Evaluate patient care based on standards of practice and compliance with the hospice plan of care., • Ensure that patient care is delivered within budget, and at the prescribed mix of general inpatient care and respite care., • Ensure timely and accurate documentation by all team members, to include timely and appropriate recertifications and completeness of patient records., • Evaluate team members per policy. Responsible for annual evaluations of assigned staff. Timely assessment of performance with timely interventions for care and performance that does not meet standards., • Communicate with other managers to ensure appropriate and adequate housekeeping, facility care, and dietary support., • Coordinate and lead team meetings. Assure that interdisciplinary care plans are updated every two weeks, recertifications are current, and physicians’ orders are received and filed appropriately., • Ensure that reports are completed as per policy, to include variance reports, infection control reports, incident reports, performance improvement reports, chart audits, and pain assessments., • Inform Hospice House Director when additional team members are needed, and assure that qualified staff are hired and comprehensive orientation is given to each new team member., • Ability to budget and monitor for cost-effective provision of appropriate patient care., • Ensure that patient surveys are mailed weekly by Team Assistant., • Participate in staff development as required, to include 15 hours of CEUs per evaluation year as per Columbus Hospice policy., • Attend and/or participate in one Memorial Service per evaluation year as per policy., • Participate in performance improvement activities and particularly monitor issues specific to Hospice House care., • Attend management and other meetings as scheduled., • Attend mandatory in-service education and staff meetings., • Attend social functions and education activities as assigned., • Ability to supervise and manage on a daily basis a professional mix of disciplines as assigned. Identify own learning needs and participate in educational opportunities to meet those needs., • Demonstrate commitment to the mission and service excellence process by adhering to all facets of the Columbus Hospice Policies and Procedures., • Represent the company in a positive and professional manner at all times with others, e.g., coworkers, management, vendors, and customers., • Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job., • When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor., • Assume duties of Hospice House Director as assigned., • Strong clinical skills, comfort with teaching, and ability to care for hospice patients and families throughout the age continuum., • Understanding of and commitment to the hospice philosophy.
