In this role, you will manage patient care plans, perform skilled nursing visits, and support patients in maintaining independence and safety at home. Key Responsibilities Skilled Nursing & Care Assessment • Provide skilled nursing visits in accordance with the established plan of care., • Perform comprehensive patient assessments (physical, psychosocial, and environmental)., • Monitor patient condition and identify changes in health status., • Collaborate with physicians, therapists, and interdisciplinary team members., • Report changes in condition and adjust care plans as needed., • Communicate effectively with patients, families, and caregivers., • Collaborate with physicians, therapists, and interdisciplinary team members., • Report changes in condition and adjust care plans as needed., • Communicate effectively with patients, families, and caregivers., • Educate patients and families on disease management, medications, and treatments., • Provide guidance on safety, infection prevention, and home care techniques., • Complete timely and accurate documentation in the EMR system., • Follow all agency policies, safety protocols, and infection control standards., • Maintain compliance with HIPAA and regulatory requirements., • Participate in trainings, case conferences, and quality improvement initiatives.Requirements:, • Active and unrestricted RN license required., • 1+ year of nursing experience required, home health experience preferred., • Valid CPR certification., • Current driver's license, reliable transportation, and auto insurance., • Strong clinical judgment and assessment skills., • Excellent communication and time management abilities., • Ability to work independently in a home-based setting., • Prolonged standing, walking and driving required., • Ability to lift, move or assist patients and equipment up to 50 pounds., • Ability to work under stressful conditions to meet clinical and documentation deadlines., • Possesses manual dexterity, hand-eye coordination, and physical stamina., • Ability to assess patient needs, make timely decisions, and address psychosocial needs of patients and families.
