Job DescriptionSalary: Position Summary ATTUF Group is seeking dedicated individuals or families inNorthern Coloradoto open their homes to support individuals with intellectual and developmental disabilities (IDD). As aHost Home Provider (HHP), you will offer a safe, stable, and person-centered home where individuals can thrive and live as independently as possible. This is acontracted 24/7 care rolethat allows for meaningful impact in someones daily life. Key Responsibilities Direct Support & Daily Care • Provide24/7 supervisionand support based on the individuals needs and level of care, • Assist withactivities of daily living (ADLs)including hygiene, meal preparation, dressing, and mobility, • Support the individual in achievinggoals outlined in their Individualized Service Plan (ISP), • Fostercommunity involvement, helping with transportation to appointments, recreation, and day programs, • Administer or assist with medications as delegated and documented, • Attend to health needs and coordinate with medical providers as necessary, • Maintain asafe, clean, and accessible living environment, • Monitor for and report any changes in behavior or health status, • Maintaindaily service documentation(T-Logs), medication administration records (MARs), and incident reports, • Participate in team meetings, ISP reviews, and evaluations, • Communicate regularly with ATTUF Group staff, case managers, guardians, and service coordinators Requirements Home Requirements • Aprivate, furnished bedroom for the individual receiving services., • Clean, accessible, and safe home environment., • Home must be single story., • The guardian is seeking a couple with a strong family environment to provide care for a 45 year old member. Members have ambulatory challenges and will require assistance with some ADLs. Member attend a Day program. Members are categorized as medically complex., • At least 21 years of age, • Must live inLoveland, Fort Collins, Greeley, Longmont, Windsor, or Wellington, CO, • Valid Colorado drivers license, reliable vehicle, and insurance, • Ability to passcriminal background checks, abuse registries, and fingerprinting, • Must be physically and emotionally capable of providing 24/7 care, • Willingness to work with individuals who may have complex behavioral or medical needs Required Training (Provided by ATTUF Group) • Host Home Orientation, • First Aid & CPR Certification, • Medication Administration (QMAP), • Therap Documentation System Training(T-Logs, MARs, incident reporting), • Mandatory Reporter Training, • Behavioral Support & De-escalation Techniques, • Rights, Abuse & Neglect Prevention, • Emergency Preparedness & Evacuation Planning, • Ongoing annual training updatesas required by state and agency regulations Compensation • Daily reimbursement rate based on client needs and level of care, • Additional funding may be provided for room and board, respite, or special supports, • Payments issued monthly upon submission of required documentation, • Access to support team, nursing services, and clinical oversight Why Be a Host Home Provider with ATTUF Group? • Make a lasting difference in someones life, • Receive comprehensive training and ongoing support, • Work from home with flexibility and autonomy, • Be part of a mission-driven team that valuesinclusion, respect, and individualized care, • Gain fulfillment through service and meaningful relationships
