Job Description Our Organization: TCC Family Health, founded in 1939, a Federally Qualified Health Center (FQHC), provides comprehensive, high quality health care and wellness services to those of all ages in the under resourced and underserved need in the greater Long Beach community. TCC is now a vibrant network of 14 clinic sites, including three school-based health centers, and one mobile medical clinic. Last year TCC served almost 40,000 patients through 175,000 medical, behavioral health, dental, optometry and wellness visits. The Care Coordinator is responsible for providing patients with appropriate referrals to internal/external resources and agencies, coordinate specialty care for clinic patients, schedule appointments for clinic patients with a provider of a physician, serve as a liaison between the Provider, Registered Nurse (RN), Licensed Vocational Nurse (LVN), and Care Team Supervisor to ensure patients receive appropriate care. Must interact in a positive manner with staff, patients, families, and members of the community. ESSENTIAL JOB FUNCTIONS: • Completes routine reports, messages, and forms, • Communicates in written and verbal form, using correct grammar, • Submits all correspondence within the requested time frame, • Relays all messages as instructed, • Organizes and files record according to established clinic procedures, • Files records in chronological and alphabetical order, • Effectively utilizes office machines to include a computer, fax, and photocopier, • Collects and distributes mail in a timely manner, and in accordance with established clinic procedures, • Participates in huddles, • Schedules appointments, • Takes messages for the Care Team, • Follows all policies and procedures, • Follows-up on failed appointments via telephone and correspondence for children with special needs., • Serves as liaison between clinic, school nurses, regional center workers, public health department and other agencies contributing to the care of clinic patients., • Works with several subspecialist physician offices to prepare paperwork and arrange for scheduling of surgeries and pre-op physicals to maximize the provision of care delivered., • Assists health department staff with follow-up on CHDP PM-160 Referrals., • Assist medical staff in the completion of patient forms (ie. DME’s, IHSS Program, DMV, Medi-Cal Exemption Forms, Etc.), • Application for Medical Alert Bracelets., • Assists patients of the clinic to call their HMO plans to switch their health plan and/or PCP., • Medical Assistant Certification is a plus, • High School Diploma or GED required, • Must have 2-4 years of medical office experience in an out-patient clinical setting., • Must have demonstrated skill in filling, use of copier equipment., • Excellent organization and communication skills with attention to detail skills is required. The noise level of the work environment is usually moderate. Workspace is in an indoor office setting, shared workspace, air-conditioned in the summer and central heat during the winter. Occasionally required to be outdoors for travel, and can be exposed to variable weather conditions.
