About the Role We are seeking skilled and compassionate Occupational Therapists (O.T.s) to join our growing team across the Bay Area. The ideal candidate is clinically strong, patient-centered, collaborative, and committed to helping patients regain independence, safety, and dignity in their daily lives at home. As an AlphaCare O.T., you will play a critical role in restoring functional independence for patients recovering from illness, injury, or functional decline. You will evaluate patients in the home environment, develop individualized treatment plans, and help patients rebuild the skills needed for daily living. You will also collaborate closely with nurses, physicians, therapists, and caregivers to ensure coordinated, outcome-driven care. Qualifications • Current California Occupational Therapist (O.T.) license required, • New graduates are welcome to apply, • Strong clinical assessment and functional evaluation skills, • Ability to work collaboratively with patients, families, physicians, and interdisciplinary teams, • Excellent communication, organizational, and documentation skills, • Valid California Driver’s License required, • Multilingual abilities are strongly valued Responsibilities As an AlphaCare OT, your role is to deliver more than task-based therapy. You are responsible for helping patients regain independence, confidence, and safety in their daily lives. Responsibilities include: • Evaluate patients in the home environment and assess functional abilities and limitations, • Develop and implement individualized occupational therapy care plans based on physician orders, • Provide therapeutic interventions focused on activities of daily living (ADLs), safety, and functional independence, • Train patients and caregivers on adaptive techniques, home safety strategies, and durable medical equipment (DME), • Monitor, assess, and document patient progress, responses to treatment, and changes in condition, • Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care, • Participate in case conferences and care coordination to ensure continuity of care, • Reassess patient needs regularly and adjust treatment plans as appropriate, • Educate patients and caregivers on safe functional performance in the home, • Maintain timely, accurate, and compliant clinical documentation, • Participate in staff meetings, in-service training, and quality improvement initiatives, • Supervise Occupational Therapy Aides in accordance with regulatory requirements, • Support discharge planning when patients reach functional goals, • Comply with all agency policies, Medicare/Medicaid regulations, and clinical standards, • Deliver care in alignment with AlphaCare’s commitment to patient time, functional independence, and clinical reliability WHY OTs CHOOSE ALPHACARE • More time with patients, not pressure for maximum visit volume, • AI-supported documentation designed to reduce burnout, not increase workload, • Operational and clinical support that allows you to focus on patient care, • Flexible scheduling across local Bay Area territories, • A multilingual care environment that reflects the communities we serve, • Leadership focused on clinical quality, responsiveness, and outcomes, • A team culture built around respect, accountability, and patient progress Benefits • Mileage, parking, and bridge toll reimbursement, • Flexible schedule, • Competitive hourly compensation, • Continuing education support, • Clinical team support structure, • Company car available Work Location In-person home visits throughout assigned Bay Area territories.
