Job Description Position Description The Care Manager has overall day-to-day responsibility for coordinating the activities of the care team for clients with complex medical and/or psychiatric conditions and for facilitating clients’ access to the full range of medical and psychosocial services efficiently and effectively. The Care Manager is mainly responsible for coordinating medical care by receiving inpatient and ER admissions of targeted clients. In addition, the Care Manager is responsible for visiting clients during inpatient stays and participating actively in discharge planning and care transition activities. The position currently follows a hybrid schedule. Follows up as required to ensure services are approved., • Responsible for providing a successful/billable core service to all clients in your caseload., • Assist in crisis intervention. WSFSSH develops, manages, and provides social services in housing for low-income persons, especially those who are older, who have experienced homelessness and who live with mental illness. Job Summary Under the direct supervision of the Area Manager, the superintendent is responsible for proper upkeep and maintenance of the building with the goal of providing a safe, well-maintained facility for residents and staff. The Superintendent will carry out all repairs and preventive maintenance tasks within the building and on the grounds. He/she will be responsible to ensure that all maintenance and custodial work is being done properly and in a timely manner. Clinical and Documentation (80%) • Provide up to 20 clients with biweekly individual case management sessions to continuously assess appropriateness of current level of treatment., • Coordinate services with medical, mental health, and other social service providers; communicate with the on-site outpatient program to ensure clients on caseload are receiving and responding to clinical care in the outpatient program while residing in the treatment facility., • Arrange and participate in multidisciplinary interventions for clients on caseload who may be struggling at the current level of care and follow up by assessing progress incrementally., • Provide evidence-based group counseling to clients in the residential program., • Complete detailed progress notes, comprehensive treatment plans, psychosocial assessments, discharge plans, and other required documentation in accordance with standards., • Maintain accurately and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS and managed care standards., • Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System., • Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the resident., • Ensure that clients’ annual medical examinations and psychiatric evaluations are completed, completing referrals as needed., • Contact the mandating source upon admission of new clients in a timely manner and provide written/oral clinical updates as needed., • Outreach to referral sources to expand the network of clients., • Attend staff meetings and training as required, as well as individual and group supervision. Job Description The Superintendent will lead all on-site construction operations for ground-up projects, ensuring work is executed safely, efficiently, and in alignment with project schedules, budgets, and design intent. This role requires deep experience in vertical construction, site logistics, and multi-trade coordination. Job Description Title of Position: Case Manager Department: SupportiveHousing Reports to: Social Service Supervisor FLSA Status: Non-Exempt Salary: $25.98 per hour Positions reporting to this position: None Purpose of the Position The Case Manager is responsible for case management services for clients in transitional housing. The Case Manager will have an assigned caseload and will be responsible for the successful integration of both the housing and employment goals of each client and ensure compliance with Win’s contractual requirements with outside Funders Responsibilities: • Conduct an initial assessment of the family within 48 hours of arrival to the residence and complete the Family Case Assessment Form, Initial Service Plan, and all initial intake documentation., • Initiate referrals for necessary emergency services., • Initiate/complete intake information (CARES data entry and supporting documentation) in the Uniform Client Chart., • Ensure CARES data and supporting documents are printed and filed in designated sections in the client’s chart., • Conduct orientation with clients and review the facility rules, clients’ rights and responsibilities, grievance/complaint process, emergency disaster procedures, etc., • Assist clients with transition to assigned residential unit and housekeeping items., • Provide comprehensive case management, including crisis intervention, counseling services and other appropriate support services., • Commit to using Evidence Based Practices (EBP); strength-based case management, motivational interviewing, trauma-informed care, critical time intervention, family-centered case management, boundary setting, and cultural-reflexivity to help clients achieve their goals., • Develop an Independent Living Plan (ILP) for each client, which will define the client’s goals (in conjunction with the transitional housing income building specialists, housing specialists, child care staff and recreational staff), and track progress against goals, make modifications, and ensure compliance with the ILP., • Provide hands-on training, coaching and support to clients, and help create opportunities for practicing skills that will enable clients to achieve their ILP goals., • Ensure that clients understand and work towards meeting their responsibilities as outlined in the Client Acknowledgment Responsibility Form (CARF)., • Maintain client data in the Uniform Client Chart., • Ensure the CARES data entry and supporting documentation is complete, accurate and current at all times., • Assist families in obtaining income and public benefits (i.e., Public Assistance; SSI, etc.)., • Establish and refer clients to appropriate medical and mental health services, education/job training programs, legal and advocacy services and other community resources as needed., • Must track referrals and follow-up each client’s progress., • Assess job readiness of clients, utilizing Standardized Assessment tools., • Perform timely and accurate input of all required client information in the Uniform Client Chart, ensure compliance with OTDA regulatory requirements and DHS contractual requirements.
