Your Responsibilities Include: • Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities., • Delivering dementia-informed, relationship-based care across the continuum of care., • Conducting comprehensive assessments and developing tailored care plans for older adults., • Managing chronic conditions and performing care with skill and empathy for patients and their families., • Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care., • Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals., • Maintaining consistency in documentation, follow-through, and patient communication. Pay Range for Posted Region$140,000—$200,000 USD
