Job Description BronxWorks helps individuals and families improve their economic and social well-being. BronxWorks has provided homeless outreach in the Bronx via contracts with the NYC Department of Homeless Services since 1995. Our outreach team provides mobile case management services to a caseload of over 400 chronically street homeless adults, with the ultimate goal of ending their homelessness. The Clinical Coordinator directly supervises the team’s case management staff and works with the Program Director to administer and sustain the program. RESPONSIBILITIES: Clinical Practice: • Serve as psychiatric liaison for the agency’s contracted psychiatric services with Janian Medical Care (currently 4 days per week), • Discuss and triage clinical updates and next steps for all clients on treatment caseload with psychiatric team and social services staff; ensure timely follow up, • Work across programs within the Adult Homeless Services portfolio to identify clients appropriate for psychiatric treatment caseload, • Prioritize emergent concerns and escalate to appropriate staff; relay communications based on guidance of on-call psychiatric provider, • Provide short-term therapeutic counseling and crisis intervention; support safety planning; conduct risk assessments and psychoeducation for high-risk clients, • Complete training and conduct 9.58 Involuntary Transports for clients requiring emergent psychiatric evaluations in a hospital setting, • Support psychiatric hospitalizations of clients by communicating with hospital staff throughout hospitalization course, advocating for additional community supports (ACT/IMT or AOT assignment) as needed, and ensuring safe discharge plans, • Facilitate trainings for all staff about clinical topics, including psychopharmacology, risk assessments, community resources, harm reduction, and hospital coordination, • Work directly with the contracted psychiatrists and nurse practitioners to coordinate the team’s caseload and psychiatric schedule, • Ensure clients are scheduled and rescheduled in accordance with medication due-dates and follow-up intervals, • Triage new appointment requests, • Ensure adequate “back-up” appointment coverage to meet attendance goals, • Compile and transmit referral documents in BronxWorks AWARDS database (ensuring they include relevant clinical updates), • Track, file, and distribute psychiatric notes, recommendations, and comprehensive evaluations to relevant team members, • Ensure the timely submission and approval of 2010E supportive housing applications that include Janian psychiatric evaluations, • Licensed Master Social Worker (LMSW), • Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points, • Knowledge and experience working with diverse and trauma-impacted communities, • A minimum of two years’ experience in social services; prior experience working with homeless populations preferred, • Strong oral and written communication skills, • Detail-oriented and resourceful with strong time management and organizational skills, • Proficiency in Microsoft Office suite and other standard business technology, • Ability to use a computer for prolonged periods, • Ability to occasionally lift and/or move up to 10 pounds, • Ability to stand, walk, or sit for long periods of time, • Ability to bend and retrieve objects and/or documents, • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation, • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work, • TB Test required within the first 120 days of employment BronxWorks seeks to build an inclusive organization grounded in respect for differences.
