Job Description Position Summary: The Philadelphia Department of Public Health (PDPH), through its COVID-19 Containment Program in the Division of Disease Control, is responsible for continued COVID-19/SARS-CoV-2 response and mitigation within Philadelphia. The program works to educate residents on COVID-19, develop policies to slow the spread of disease, provide infection control guidance to institutions, and work with partners to address other ongoing public health issues related to COVID-19 in the City of Philadelphia. A component of this effort involves partnerships with Early Childhood Education (ECE), Primary, and Secondary Education settings to support mitigation strategies. The Pediatric Response Specialist will conduct outbreak investigations for COVID-19 cases in K-12 Schools and ECE settings. This position will investigate outbreak reports to understand the severity of the illness and determine its classification and burden on the setting. This will involve reaching out to school nurses and administrators, following up regularly with all parties to ensure they are implementing best public health practices to prevent the spread of COVID-19, communicating with ACD for appropriate reporting of other communicable diseases, supporting personal protective equipment (PPE) and at-home test kits distribution to K-12 Schools and ECE settings at PDPH storage facilities, and visiting schools and early childhood education centers throughout Philadelphia to review existing COVID-19 mitigation strategies and provide guidance for improvement during respiratory health season. The programming builds upon youths’ resiliency and sense of self-worth for positive outcomes and redirection. There are no traditional office hours. Case Managers will work primarily in the afternoon and evenings, specifically during the school year and at weekends to provide maximum involvement with the family and the identified clients. There may need to be a need to start early to help a participant get to school. The Case Manager will be placed on an on-call roster and will be responsible for providing emergency assistance to clients assigned in the program 24 hours a day while on duty. At crisis times, there may be a need to “double team”., • The maximum caseload is 17 clients. Case longevity is determined by court and or DHS personnel., • The Case Manager’s role is to be able to report to the court that identified issues have been resolved through a collaborative effort with school personnel and community resources and through the site-based programming activities., • Case Manager must meet with the supervisor weekly for supervision., • Case Manager must meet with the family and see all children once each week, and must coordinate work with others involved with the family, possibly including parent aides. In this way, families can access our staff round-the-clock., • Case Manager will ensure that services and counseling are commensurate with identified issues, and may need to be increased beyond minimum standards., • Following assessment of family’s identified needs, Case Manager will identify community resources, and work collaboratively with the Counselor to assist families with linking them to the appropriate community resources., • Case Manager is responsible for ensuring that school age children receive the appropriate services that meet their educational needs., • Case Manager will ensure that he/she keeps accurate documentation specifically all unsuccessful efforts to maintain the expected level of intensity and monitoring. All documentation and communication will be directed to the assigned DHS worker or court personnel via e-mail correspondence, telephone calls, and with hard copies to be kept in client’s file., • Case Manager will facilitate the family’s transportation for appointments with other service providers by providing directions, tokens, as needed, and accompanying on public transportation or driving them to appointments and meetings in their own vehicles., • Case Manager must attend monthly team sessions to determine and monitor service appropriateness for each youth and to inform the team of the client’s adherence to orders of the Court and to the program., • Case Manager must attend all mandatory IPS trainings and activities hosted by the Department of Human Services or designee., • Participates in meetings to ensure priorities are clear, coordination is good and communications are open., • Cooperates and communicates as a multi-discipline staff team member through formal meetings, informal discussion and other participation as necessary., • Models traits of a responsible team member, executing job responsibilities, open communication, good follow-through; supports team members to do the same. This JOH Project will also be hiring 3 Peer Stipend Worker (PSW) positions to offer paid internships to JOH alumni seeking to re-enter the workforce and explore becoming a full time CRS. This position will work closely with JOH supervisory staff to lead, guide, and coordinate the provider CRS positions, as well as the PSW positions, as well as support current participants and alumni of the JOH project in their overall recovery process. Education and Experience: Master’s degree in (including but not limited to) sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, education. The Creative Design Specialist is responsible for creating high-quality graphic design that strengthens brand identity and communicates messages effectively across digital and print platforms. This role requires strong visual design skills, creative problem-solving, and the ability to manage multiple projects from concept through completion while collaborating with colleagues and stakeholders.
