Job Description Summary: Case Managers provide comprehensive services to individual clients and families residing in cluster apartments. Services provided by Case Managers include assessment, case planning, developing an Independent Living Plan, case facilitation and management, counseling, referrals, information workshops and support groups. The Case Managers coordinates with other program staff and local agencies to provide short and long term care necessary to meet the needs related to moving families to permanent housing. Essential Job Functions: The Case Manager is responsible for managing a caseload of families and children residing in the shelter. Provides counseling intervention services to residents and coordinates all client services as identified in the Independent Living Plan (ILP). Major Duties and Responsibilities: • Intake and assessment for families referred to the shelter., • Manage a caseload of families referred to the shelter by DHS., • Conduct homes visits at least twice a week to meet and discuss resident’s Independent Living Plans., • Conduct bi-weekly progress meetings with residents to assess progress., • Provide crisis intervention and short-term individual counseling., • Providing information and counseling to families on services and planning to move into permanent housing., • Advocate on behalf of clients for public assistance and entitlements with city agencies., • Case management, preparation of case records, prepare monthly and quarterly statistical reports., • Mediation for internal conflicts in the living quarters., • Group facilitation and conducting of educational presentations for communities., • Provide updates as appropriate and assist clients in reporting any challenges to moving to permanent housing., • Serve as an advocate and liaison between the family and various agencies, such as HRA and ACS., • Maintain routine administrative responsibilities to include the preparation of correspondence, maintaining, files, mail, email, and telephone. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. In all programs, we provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. We want all children we support to move forward with their lives and thrive. About the Role The Clinical Supervisor oversees the clinical services of the UC program. We are seeking a Clinical Supervisor with a license to provide clinical services (i.e.: LMHC or LMSW), and we are invested in supporting their continued professional development by providing clinical supervision towards further clinical licensure. What You’ll Do · Oversee all clinical services provided within the day program including individual/group supervision, mental health assessments, crisis intervention and discharge planning. · Provide direct oversight and facilitate individual and group supervision to the clinicians on caseload. · Review and approve clinical notes in MyEvolv to ensure quality and timely service provision. · Ensure comprehensive assessments are conducted using standardized measures as per standards set forth by Office of Refugee Resettlement (ORR). · Provides temporary coverage for clinicians as needed for individual, group therapy and crisis management. · Participate in monthly staff development for clinical team and stays up to date with effective modalities and therapeutic services. · Track compliance and quality assurance for the clinicians on caseload. · Provide clinical insight and guidance to the clinicians on evidence interventions particularly for cases with mental health and/or psychiatric complexities. · Work closely with the Case Managers Supervisors to ensure a safe reunification process for all minors in care and ensure both Case Management and Clinical teams are working in tandem to thoroughly assess and vet children and sponsors. · Oversee timely and accurate data submission for quarterly reports for the clinicians on caseload. · Maintain regular communication with and coordination of services with other members of the multidisciplinary team and stakeholders as needed (ORR, GDIT, LSP, Young Center). · Attend weekly staffing with ORR and GDIT to be up to date with all cases and provide clinical support and guidance to the team as needed. · Conduct outreach and connection to secure additional mental health resources in the community to strengthen and provide external services to the community when needed for specific cases. · Along with the Prevention of Sexual Abuse (PSA) coordinator reinforces the Zero Tolerance Policy, ongoing psychoeducation and ensure a safe environment for all children in care. · Ensure that Child Level Events are thorough and are submitted on a timely manner according to ORR standards. Maintain timely and ongoing communication with ORR DHUC for complex cases and clinical recommendations. · Participate in treatment team meetings and other meetings pertaining to the needs of the children in care. · Complete internal audits to ensure that the clinical records meet the requirements of standards set by OCFS and ORR. · Assist in facilitating training and shadowing for new clinicians. · Maintain confidentiality of sensitive and protected information according to HIPAA standards. · Integrate a collaborative approach with all different departments in the program to ensure clear communication, quality service provision and teamwork to meet the needs of the children in care. · Demonstrate commitment to departmental and organizational goals. · During times of influx, the lead clinician could be temporarily assigned a small caseload if needed to support the program. · Assumption of other duties as assigned. Job Description Position: Mental Health Clinician, Safe Options Support (SOS) Function: Community outreach on the streets and subways, coordinating participants needs before and after their move from street to home, enhancing their daily living skills, providing supportive counselling, and advocating on their behalf when faced with discrimination or healthcare inequities. Job Description PRIMARY FUNCTION/PURPOSE: The core responsibility of the Residential Aide (RA) includes the continued assurance of support and care to clients residing in the shelter. Under the general direction of the Shift Supervisor, the RA provides direct assistance, support, and care to clients residing in the facility, as well as supporting, assisting, and enforcing facility rules.
