Job Description Candidate must be a licensed or certified CNA, MA, or LVN. Position Summary The TCC is responsible for technical skills encompassing patient care and related tasks under the direction of a Doctor, Physician’s Assistant and or Nurse Practitioner. The TCC works closely with patients and medical staff to ensure proper use of equipment and telehealth visit is accomplished. General Duties and Responsibilities The essential functions include, but are not limited to the following: • Delivering direct patient care to patients as assigned, using established procedures, • Documenting history; recording and reporting pertinent observations and patient reactions to staff, as appropriate. documenting lab procedures and ensuring follow up on results, • Using screening procedures, including medical history review, and observing and documenting patient vital signs as required, • Maintaining quality results by running standards and controls, • Verifying equipment function through routine equipment maintenance and advanced trouble shooting; calibrating equipment utilizing approved testing procedures; monitoring quality control measures and protocols, • Training other TCC’s or other workers The essential functions include, but are not limited to the following: • Acknowledge receipt of scheduling email with the administrative team. Ensure consent forms are in place before scheduled visits., • Introduce yourself to the patient each visit., • Explain consent forms and obtain the patient's or legal guardian's signature on the forms., • Obtain the patient's pharmacy information., • Take patient's vital signs and document for provider., • At the designated time, contact the provider to initiate an evaluation., • Support phone/iPad for the patient's for duration of the evaluation., • Once the evaluation is completed, TCC will end the communication with the Clinical provider, thank the patient for the evaluation and leave the patient in a comfortable position before leaving the patient's home and or SNF bed., • At no time will the TCC offer his/her services to a patient, Agency, or SNF, that they have provided services to or been introduced by PAS. Any infraction of this policy is a term for immediate discharge from employment., • TCC dress code will be nursing scrubs. Appearance and personal behavior will always be professional when dealing with patients, family members, agencies, or skilled nursing facility staff. The noise level in the work environment is usually moderate. The employee may be exposed to some hazards such as radiation, bodily fluids that may contain disease, and fumes from laboratory chemicals. Preferred Qualifications (Knowledge, Skills, and Abilities) • High school diploma or equivalent combination of education required, • CNA, LVN, MA License – must be current in CPR certification., • Accredited college degree or certificate highly desirable, • Knowledge of healthcare regulations and preventative measures (HIPAA confidentiality, infection spread, etc.), • Exemplary patient care experience and job knowledge, • Excellent oral and written communication skills, • Ability to prioritize and manage time effectively; ability to critically think, • Knowledge of quality control, • Ability to exercise manual dexterity, control precision and arm-hand steadiness accurately, • Ability to be empathetic with patients; ability to maintain confidentialityCompany DescriptionPost Acute Specialists is a multi-specialty group dedicated to improving access to medical specialists through telehealth.
