This position involves providing supervision to up to 6 six adolescents who are in residential treatment for 30-60 days for issues related to mental health and substance abuse. Residential Counselors support clients in their daily activities and facilitate groups with our clients. Residential Counselors must be patient, resilient, reliable team players, and must know how to maintain appropriate boundaries. Position Summary The Medical Coordinator Assistant is a delegated clinical support role responsible for assisting with client health support, care coordination, and documentation for clients enrolled in Whole Person Care Clinic programs, including Recuperative Care. This role operates only under the direction and supervision of authorized clinical staff and supports health stabilization, recovery, and continuity of care within scope of practice per program guidelines. Job Summary: The Lead Care Manager (LCM) role involves developing personalized care plans, coordinating member services, and collaborating closely with members and families, as well as Primary Care Providers to ensure they receive necessary medical treatment and support.
