Job Description Job Description: Family Advocate & Community Liaison JOB TITLE Family Advocate & Community Liaison PROGRAM PuroPREP PLUS REPORTS TO Program Manager SALARY $55,000 LOCATION Houston, TX Hybrid JOB TYPE Non-Exempt WORK SCHEDULE Full Time General Description The PuroPREP PLUS program equips youth with the relational knowledge, skills, and tools to help them successfully transition to adulthood by implementing interventions that result in positive effects such as socio-emotional skills, prevention of risky behaviors, development of healthy relationships, among others. These services are offered in Harris County, Texas in partnership with community faith-based organizations About You The ideal candidate for our program is: • A self-motivated and dedicated person who is excited and passionate about helping youth learn ways to build healthy relationship skills for now and the future while supporting positive socioemotional development and successful transition to adulthood., • You are a personable, energetic, resourceful, and empathetic team member who can cultivate supportive relationships with youth to gain trust and enhance CCM effectiveness and carry out projects in a prioritized manner to meet our internal Urban Strategies goals for PuroPREP PLUS Standards., • Excellent communication and interpersonal skills., • Ability to work independently and as part of a team., • Attention to detail and ability to maintain accurate records., • Adapt to unexpected circumstances, think critically and is resourceful., • Ability to work well under pressure and manage multiple tasks simultaneously., • Adapt to frequent changes and bring new ideas and innovations to the workplace., • Foster a harmonious work environment and find mutually beneficial resolutions that promote teamwork. Minimum Qualifications • Associate or bachelor’s degree in social work, Human Services, or -in a related field., • One to three years’ experience working with at-risk or low resource youth in community programs., • Excellent oral and written communication skills; Advance MS Office Suite., • Fluent in English and Spanish (verbal/written)., • Working knowledge of local community resources for youth in Harris County., • Must have Valid Driver’s License, Personal Vehicle, and Liability Insurance required. Preferred Qualifications • Experience in youth development programing, • Bachelor’s degree in organizational management/business administration/public administration and or a minimum of 4 years' experience of position responsibilities and qualifications stated above preferred. What You’ll Be Doing • Manage a caseload of assigned participants; intakes, assessing participant needs, coordinating referrals, and ensuring follow-up to support access to services., • Develop Individual Development Plans (IDPs) to meet the unique needs of youth, offering guidance that reflects their distinct roles and circumstances, • Makes connections to community resources supporting participants’ needs, • Provide support to Implementation Partners and identify CFBOs with the potential to become IPs., • Provides supplemental workshops to youth, • Engage in comprehensive, integrated planning with the management team:, • to create a system of services that support the eligibility, recruitment, selection, enrollment, and attendance of youth participants, and that is responsive to the results of the community assessment., • to create youth and community engagement goals/outcomes and a system of services that align with the PuroPREP PLUS Standards., • to support the Lead Family Advocate to develop policies and procedures for the PuroPREP PLUS Youth/Community Engagement system of services., • Assist in the assessment of community, program, youth needs, strengths, and resources in quality-of-life issues; compile and analyze data; produce reports; assure the integration of dual language learners, homeless youth, youth in foster care, and youth with disabilities., • Under the direction of the Lead Family Advocate, assist in tracking, reporting, each youth's attendance and follows through with individual intervention in situations of chronic absenteeism., • Assist with the development of the PuroPREP PLUS Assessment. Assist with the annual program self-assessment and federal onsite review process, focused on compliance with PuroPREP PLUS and progress on youth/community engagement; report on strengths, recommendations, required improvements; implement improvements and recommendations; incorporate into the program planning process., • Assist in creating the criteria for the selection and enrollment of youth assuring responsiveness to community assessment and reflecting data on dual language learners, homeless youth, youth in foster care, and youth with disabilities., • Perform intake/registration procedures for all youth and ensure applications are complete and data is accurately entered in the database., • Assure and maintain record keeping and reporting policies for waitlists, enrollment, attendance, timelines, schedules, and procedures in accordance with the following designated state and federal program requirements., • Conduct ongoing monitoring, tracking, follow-up, and analysis of enrollment and attendance data, and produce regular reports for management meetings., • Maintain enrollment forms, ensuring information is current, correct, and disseminated to all necessary staff to meet program requirements., • Perform recruitment activities in response to enrollment data and waitlist data., • Ensure full program enrollment and maintenance of a sufficient waitlist., • Compile and submit Program Information Report (PIR) data periodically.Youth/Community Partnerships, • Implement the system for Youth and Community Engagement Services., • Implement a system of services, record keeping, and reporting of the Family Partnership Agreement and assure its alignment to the PuroPREP PLUS Standards., • Assist in being the program resource for consultation, information, and referral regarding community resources to staff and client families., • Completes family service home visits and Needs Assessments PHYSICAL DEMANDS: • Ability to see at normal distance, hear normal conversations and sounds and use hands and fingers to fill out required forms. Ability to lift occasional lifting of thirty (30) pounds is required and must be able to push/pull up to thirty pounds horizontally.
