Overview Coordinates and provides high quality nursing care to Hospice patients and families consistent with Hospice Care philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. • Assesses the physical, social, psychological, living conditions and pastoral care needs of the patient/family through professional knowledge and skills of observation, interviewing and physical examination on an ongoing basis., • Establishes and revises, as necessary, the nursing component of the patient/family plan of care based on goals mutually acceptable to the patient/family and significant others and makes referrals to other services, as necessary., • Coordinates implements and evaluates the effectiveness and outcome of the care plan by ongoing assessment, planning, coordination, counseling, teaching and supervision., • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient., • Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations., • Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care., • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals., • Visits clients every two weeks to assess the clients’ condition and update the plan of treatment., • Monitors the effect of the plan of cares on the patient/family and informs physician and other personnel of changes in the patient's needs., • Manages a caseload of patients., • Provides nursing care services and initiates appropriate preventive and rehabilitative nursing procedures., • Participates in interdisciplinary team meetings., • Appraises care provided and revises the goals and approaches to patient/family care in coordination with the interdisciplinary team., • In coordination with the interdisciplinary team, assesses the family's needs during the bereavement period and provides ongoing counseling with referral to community resources, as appropriate., • Initiates and maintains written (including clinical and progress notes) and verbal communications to assure optimal patient care., • Understands and interprets Hospice Care reimbursement policy and determines the billing source for patient care., • Maintains productivity sufficient to meet Hospice goals., • Participates in the orientation of new staff., • Orients, instructs and supervises License Practical Nurses, paraprofessional staff and volunteers., • Consults with and educates the client and family regarding the disease process, self-care techniques, and prevention strategies., • Performs established work procedures in relation to Region/Program workflow., • Understands and interprets the Hospice philosophy and responsibility to other health professionals, laymen and community., • Assumes responsibility for professional growth., • Evaluates conformance of the inpatient facility in carrying out the interdisciplinary team's plan of care for select patients, when assigned., • Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: License and current registration to practice as a Registered Professional Nurse in New York State required Education: Associate's Degree in Nursing, with at least one year of experience as a full-time staff nurse in a medical-surgical nursing area or other clinical healthcare experience required or Bachelor's Degree in Nursing from a State-approved diploma program required Work Experience: Minimum of one year of clinical nursing experience including at least six months of Hospice, or Med/Surg or ICU or Emergency Department or Home care, Palliative Care, Oncology or Long Term Care experience required Demonstrated computer skills may be required based on operational needs Pay Range USD $118,942.00 - USD $152,717.00 /Yr. The Division of Cerebrovascular Diseases led by Colum Amory, MD, MPH, at the Department of Neurology, Hackensack University Medical Center (HUMC), a major academic medical center within the Hackensack Meridian Health network (HMH), seeks a full time Vascular Neurologist to join its Joint Commission certified Comprehensive Stroke Center. The successful candidate will have completed a residency in Neurology and an ACGME accredited fellowship in Vascular Neurology. HUMC is one of the 3 principal teaching hospitals for the Hackensack Meridian School of Medicine and for the HMH network. It features 775 beds and sees some 900 annual stroke admissions, many transferred from its affiliated spoke Primary Stroke Centers. The stroke unit is staffed by APNs 24/7. Stroke APNs provide support to the stroke service on all units. The Department of Neurosurgery has a highly experienced team of neurosurgeons and interventional surgical neuroradiologists providing care to patients with large vessel occlusions, intracerebral hemorrhage and subarachnoid hemorrhage. The neurocritical care program runs a fully staffed and closed Neurocritical Care unit. Night call is facilitated by the availability of telemedicine robots to all members of the stroke service. Outpatient stroke clinics provide consultation for complex stroke diagnoses as well as follow-up for discharged patients. Teaching responsibilities include stroke neurology training of medical students and neurology and other residents. The Cancer Genetics Program at the Northwell Health Cancer Institute is looking for an additional adult cancer genetic counselor who would like to be a part of a vibrant and growing clinical, research and educational environment. Position is based at Phelps Hospital in Sleepy Hollow, NY. The position will be a mix of in-person (approximately 3-4 days/week) and remote coverage (approximately 1-2 days/week). The successful candidate will join a team of eight adult cancer genetic counselors, two clinical research assistants, an administrator and a dedicated scheduling team. In addition to providing clinical care, the counselor will have the opportunity to supervise genetic counseling students, assist in medical student and resident education, and participate in clinical research. The genetic counselors at Northwell Health are an integral part of a multidisciplinary team that includes medical oncologists, surgical oncologists, advanced care practitioners, and psycho-oncology professionals.
