Job DescriptionDescription: QRM is seeking a part time Telehealth PT, for a PRN or part time position (approx 5-10 hours/week). Candidates must be licensed in multiple states. The Telehealth Rehabilitation Associate is a member of the Quality Improvement and Education (QIE) team who provides therapy services to patients remotely. The Telehealth Rehabilitation Associate reports to the Director of Quality Improvement and Education. Key Results Area • Supervise Therapist Assistants and therapy students in direct patient care to carry out plan of care and patient-related activities per the state practice act and reimbursement regulations, • Provide high-quality, patient-centered therapy evaluations, treatments, and discharges via telehealth platforms, • Deliver skilled interventions aligned with Medicare and payer regulations, SNF policies, and professional practice standards, • Document timely, accurate, and comprehensive therapy notes in the electronic medical record (EMR) in accordance with regulatory and professional guidelines, • Engage patients through interactive, functional therapy activities adapted for virtual platforms, • Support facility staff through caregiver education, recommendations for equipment or environmental modifications, and interdisciplinary coordination, • Adhere to all telehealth best practices, HIPAA regulations, and facility protocols for remote service delivery, • Participate in case conferences, care planning meetings, and team communication as appropriate, • Demonstrate a positive attitude and a collaborative team-building approach with all patients, clients, and team members, • Attend QIE educational webinars as requested, • Respond to email communication within four business hours and follow up by the deadline, • Evaluate and assess personal performance on a routine basis, • Licensed Physical therapist (Compact Licensure preferred) with a minimum of 5 years of experience in LTC. Responsible for maintaining license requirements in all states they are providing services, • Proficiency in federal and state reimbursement, regulatory and quality programs, • Independent and self-motivated, • Ability to provide information in a concise manner, • Collaborative and detail-oriented professional with strong analytical and organizational skills, • Strong proactive written and verbal communication supporting client satisfaction and retention, • Strong critical thinking and clinical decision-making skills, • Strong time management with a proven ability to meet deadlines, • Function well in a fast-paced and at times stressful environment, • Demonstrate discretion, sensitivity, and ability to maintain confidentiality, • Ability to establish and maintain positive relationships, • Function independently, have flexibility, personal integrity, and the ability to work effectively with patients, clients, and team members, • Microsoft Office and EMR proficiency, with the aptitude to learn new programs
