Job DescriptionDescription: DESCRIPTION · Provide on-site management of the Resurgens Orthopaedics Durable Medical Equipment (“DME”) Program. · Under the direct supervision and orders of the physicians, provide proper fitting and education of orthopaedic DME products for patients, gather necessary paperwork to facilitate billing on behalf of Resurgens Orthopaedics, and collect payments at time of service as directed. · Provide the highest level of customer service to patients and their family members, physicians, clinic management, and all other employees. · Assist with education of clinical employees on all aspects of the DME Program to cover patient volumes and days off. Meet performance goals of Resurgens Orthopaedics. · Properly fit and provide education to patients on the application, use, care, and expected outcome for DME products as indicated by the prescribing physician instructions and manufacturer recommendations. · Educate patients on their insurance plans and provisions, financial responsibility, and collection of patient out-of-pocket portion. · Create claims for dispensed DME. · Document and reconcile inventory and monthly billings. · Utilize practice-assigned inventory management software to manage daily operational tasks, including but not limited to: patient demographic and insurance information, pre-certification requests, inventory management, purchasing, reporting, patient encounter reconciliations, and CPM rental tracking and maintenance logs. · Ensure that all necessary documentation is obtained as it relates to third party payer requirements and standard operating procedures. · Obtain and track pre-authorizations and communicate status with patients and providers as necessary. · Conduct monthly inventory counts and report discrepancies as found. · Ensure proper reports, records, policies, and procedures are maintained relative to the performance of required daily activities. · Develop and maintain clinic-specific Operations Manual for the DME Program. · Perform patient scheduling for DME product pick-up, fitting, education, order processing, delivery (if applicable), and courtesy call back. · Participate in training of new clinical employees, if applicable. · Comply with all elements of HIPAA law, Medicare, Medicaid, and other required programs. · Participate in any additional training or in-services required by the practice. · Coordinate vacation requests with assigned DME Supervisor and/or DME Director and assist with arranging coverage. This position is hybrid, and candidate must be located in the state of Georgia. The Review Nurse, UCR & Medical Claims Nurse is a member of a professional multi- disciplinary work team, and responsible for the analysis and monitoring of policy compliance for Medicaid providers. This position will determine an estimate of recoverable amounts by reviewing coding and billing patterns and identifying payment errors of these previously identified providers for the Department of Community Health (DCH), Office of Inspector General, Program Integrity Unit. Position is also responsible for conducting medical claim reviews of suspended, pre-pay and reconsideration/appeals/provider inquiries/retrospective claims that require a medical review prior to adjudication. This position is hybrid (office/onsite and remote). In this role, the ideal candidate will do the following: • Conducts on-site or desktop reviews through claims analysis. Review levels include initial, corrective action plan (CAP), and administrative., • Inform providers in advance of upcoming on-site visits. Conducts entrance and exit interviews with providers informing them of purpose of visit; on-site targeted and non-targeted reviews for the specified Medicaid providers, which includes some but not all of the following: entrance and exit sessions with the appropriate management and clinical staff from the provider location, staff interviews, and scanning of document(s) to be reviewed., • Initiates case activity log in developing the appropriate approach for the on-site review., • On-site facility tours and adhere to DCH policies., • Performs member assessments/reviews of services provided and billed either on-site or telephonically., • Make recommendations based on medical judgment and experience for the necessity of the services and the appropriateness of the setting while substantiating recommendations with clinical rationale., • Analyze, interprets and documents appropriate determination of estimated recoverable amounts from specified Medicaid providers through review and identification of policy compliance, coding/billing patterns and payment errors., • Prepare timely, accurate, written letters to providers/DCH on initial and final desktop review and on-site findings., • Correction Action Plan (CAP) review of the deficiencies identified during the initial review. The provider should include a plan to correct the issues and a target date., • Perform administrative reviews if requested by the provider. Review additional information related to requests for reconsideration after initial findings and makes a recommendation based on medical judgment and experience for the necessity of the service and the appropriateness of the setting., • Provide support and expert testimony at Administrative Law Judge Hearings as requested by DCH’s Legal Services in support of Administrative Review findings., • Serve occasionally on panel of peers to provide medical expertise regarding standards of medical care., • Provide assistance in preparing referrals for submission to the Medicaid Fraud and Patient Protection Division (MFPPD)., • Case Management Reviews of Independent Care Waiver Program (ICWP), Community Care Service Program (CCSP) and Service Options Using Resources in a Community Environment (SOURCE) programs., • Home Community Based Services (HCBS) Quality Reviews of the waiver programs for adhere to the DCH State Transition Plan., • Adjudicate suspended claims in the Gainwell Interchange system that require a medical review to ensure quality of care and appropriateness of services based on current standards of care., • Adjudicate claims for providers placed on pre-payment review., • Process Medical claim appeals/provider inquires in the Alliant appeal/provider inquiries system that requires a medical review of a system denial edit or an appeal of a denial adjudicated claim, to include dental appeals., • Perform reviews accurately and efficiently within time requirements as specified and performs accurate data entry of required review-specific information., • Evaluate submitted information and review claims that meet criteria and determine up-coding; make recommendation on appropriate level/ code based on record documentations., • Render appropriate decisions based on the information/clinical documentation received using the established criteria; and clearly and concisely documents the rationale for all decisions rendered., • Utilize Gainwell Interchange claims system to resolve suspended claims for adjudication and utilizes Alliant appeal/provider inquiry system to resolve reconsiderations/appeals/provider inquiries., • Work in collaboration with Medical Review Team to oversee and support surveillance utilization review functions., • Identify and documents suspected cases of fraud and abuse to Supervisor, for further referral to Deputy Director, Program Integrity, and Medical Review., • Working knowledge of ICD-10-CM, CPT codes, and InterQual criteria preferred., • Strong organizational skills with ability to demonstrate the work priorities., • Demonstrated ability to perform work with considerable independence by use of creative thinking, thorough analysis of problems, and use of innovative approaches to problem resolution., • Computer literate with intermediate level knowledge of Microsoft Office (Word and Excel) and proven ability to type 35 WPM or more., • Excellent interpersonal, written, and verbal communication skills required., • Excellent telephone skills, including ability to handle difficult calls/callers. Job Description POSITION: Care Partner TITLE OF IMMEDIATE SUPERVISOR: Health and Wellbeing Director DEPARTMENT: Resident Care STATUS: Non-Exempt/Hourly SUMMARY This position will provide personal care service and engagement with residents and perform related services at the community in accordance with the plan of care assigned by the primary licensed nurse. This position reports directly to the Health and Wellbeing Director. QUALIFICATIONS AND EDUCATION REQUIREMENTS • High school diploma or GED, • Successful completion of a state approved nursing assistant training program preferred, • Current, active certification on the Georgia Nursing Assistant Registry preferred, • Must successfully clear a background check, TB skin test or x-ray, and health screening, • Current CPR certification, • Current certificate of training for successful completion of the five (5) hour initial fire safety training provided by the Rules and Regulations of the Georgia Safety Fire Commissioner PRINCIPLE DUTIES AND RESPONSIBILITIES • Provides assistance with assigned personal care tasks that meet resident physical and emotional personal needs consistent with the Orchard philosophy- For example: physical transfers, personal hygiene tasks such as mouth care, bathing and dressing, activity support, guidance and direction, and most importantly the ability to make a personal connection and engage respectfully, compassionately and meaningfully, • Assists or leads small groups with engaging in one on one experiences with residents- Activities include socialization, personal growth, exercise, music, community integration, spiritual development, recreation and performance improvement under the direction of the Engagement Director, • Responsible for assigned resident care tasks, ensuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards, • Resident care task includes but are not limited to maintaining personal hygiene, including bathing, oral care, and skin and nail care, hand washing, hair care, shaving, dressing, weighing, and bed making, • Answers residents' call signals (Is the first responder to resident calls), • Assists with toileting, including bowel and bladder retraining and perineal care, • Observes residents' conditions, measuring and recording food and liquid intake and output as needed and vital signs, and reports changes to professional staff, • Provides resident care by applying dressings and supervising exercise routines, • Provides residents help with walking, exercising, and moving in and out of bed, • Performs and assists with mobility, transferring, ambulation, positioning and range of motion, • Serves and collects food trays., • Uses appropriate and safe body mechanic techniques when lifting and/or transferring resident or supplies, • Transports residents to and from dining room using a wheelchair when applicable, • Uses aseptic technique for purpose of infection control; this includes good hand washing techniques and infection control policies, • Answers phones, directs visitors, and delivers messages to residents and staff members maintaining confidentiality of all resident care information and assures residents' rights and privacy are protected at all times, • Assists in orienting resident to environment and routines upon admission and throughout residency as needed, • Documents accurately all care given, including dietary intake and output, if necessary; temperature, weight, and pulse, • Reports to nurse or supervisor for written and verbal assignments, which include daily use of Personalize Service Plan for assigned residents before initiating care, • Participates in the planning of resident care through shift reports and resident care staffing meetings and informs verbally and/or submit in writing any observations and changes in a resident for the licensed nurse to evaluate, • Encourages/assists residents in keeping their rooms clean and orderly;
