Job Description Job Title/Position: Patient Care Coordinator Reports to: Clinical Manager JOB DESCRIPTION SUMMARY: The Patient Care Coordinator’s (PCC) primary responsibility is to provide administrative support for the Clinical Managers, clinical care staff and patients. The PCC will coordinate daily/weekly/monthly clinical reports, patient supplies and interagency/community referrals. The PCC will also collaborate with agency departments, facilities and physicians as needed to coordinate appointments and/or obtain necessary documents required for care delivery and billing. The PCC must be clinically licensed & have the ability to receive a physician order. Unresolved variances will be escalated to the Clinical Manager., 5. Assists with care coordination i.e. DME, medical supplies, coordination of appointments or referrals to outside agencies., 8. Facilitates patient care coordination documents with acute and post-acute facilities during patient transfers., 9. Assists with education & training of field staff., 10. Other duties as assigned, may cover for field staff in situations of high census etc. POSITION QUALIFICATIONS 1. One-year home care experience or experience in a facility setting., 2. Must be a licensed clinician with current, active license in good standing in the state the agency is located., 3. Skilled in making effective verbal and written communications with subordinates, co-workers, consumers, referral agencies and comfortable with conflict resolution skills., 4. Ability to educate consumers on all service lines and present to seniors and aides in a group setting, 5. High energy level and passionate about care delivery, 6. Must have excellent organizational skills and ability to complete competing priorities, 7. Must have thorough understanding of home health or hospice qualifying criteria and coverage guidelines, 8. The Director will play a pivotal role in developing and executing Commit’s EC-12 policy strategy, most notably within the foundational years (ages 3-8), by leading legislative communication, engaging state agencies on rulemaking, and coordinating policy coalitions. This role is ideal of a professional with at least 4 years of direct experience in public policy, advocacy or governmental relationships, preferably in Texas, who is eager to translate local and regional insights into lasting statewide impact. The annual salary range for this role is $110,000 - $139,000. Strategize and direct all departments' involvement in third party events. Supervise all event logistics and operational matters for the arena's major tenants (Dallas Stars, Dallas Mavericks). Develop strategies from said analysis to present to EL for strengthening giving trends, celebrating churches with positive trends, and connecting to churches with negative trends., 5. Develop, implement, and coordinate recognition of top giving churches by total dollars and as a percentage of undesignated gifts. Recognition of top giving churches should occur at churches, at the Annual Meeting, special recognition meals, or at Associational meetings., 6. Connect TxB churches with approved resources for congregational stewardship development., 7. Build relationships with top giving churches (and those that have the potential to be), by amount and percentage, on a regular basis., 8. Develop a communication plan for Area Reps, Church Starters, & all other consultants to discuss talking points and provide materials to leave with every church visited. This should include stories about what is being accomplished with CP. Stories should be refreshed on a regular basis., 9. Develop and implement Cooperative Program educational information and materials for use by staff with churches., 10. Update and educate all BGCT staff on the Cooperative Program, Biblical stewardship, and the importance of the Cooperative Program to the mission of the BGCT., 11. They will provide high-level administrative and project support for the diverse activities, including, but not limited to, special events, scheduling, and producing meetings. The successful candidate is incredibly detail-oriented, produces high-level work in a fast-paced environment, and exercises discretion and diplomacy in all interactions. The Development and Administrative Associate position is highly collaborative with local staff, national development team members, philanthropic partners, and ASP leaders.
