Job Description The Clinical Care Manager (CCM) Specialist - Children’s Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care. As a member of CBH’s Complex Care team, training and certification in High-Fidelity Wraparound, an evidence-based care management intervention, will be provided. The CCM Specialist will also be responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, and treatment quality monitoring. As a member of the Children’s Complex Care team, you will have an opportunity to apply your understanding of behavioral health and your cultural competence to work collaboratively with CBH members, their caregivers, their supports, other professionals, and community partners to screen/assess, and develop individualized, family-anchored, comprehensive, and strength-based plans of care. The CCM Specialist must utilize clinical knowledge of best practices and evidence-based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, CCM Specialists consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. As a High-Fidelity Wraparound (HFW) Facilitator, the CCM Specialist assists members and their families in learning to accomplish goals they have identified by exploring the strengths and culture around each need they have. The CCM Specialist facilitates monthly interagency planning meetings to assist the member and their family in planning around and working towards meeting their needs. The CCM Specialist will transfer their Wraparound skills to the member and their family, by teaching them to think outside of the box and use natural and community supports. Essential Functions: • Work directly with members, natural supports and providers in the community, including in members’ homes., • Facilitate care coordination. Using an inter-generational approach, with system partners to address social determinants, barriers to wellness, and other needs of members, including connection to appropriate community-based services., • Create and maintain a team of system partners and natural supports for each member and their family., • Complete an effective Crisis Prevention Plan in collaboration with each assigned member and their team, • Be knowledgeable about behavioral health care services, system partners, service continuum and established medical necessity criteria and apply that knowledge in all work activities., • Attend up to 3 hours of group or individual skills-based coaching/supervision per week, • Complete Utilization Management activities including but not limited to: interagency meetings, social determinants interviews with members, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage, within established timeframes., • Authorize treatment when Medical Necessity Criteria is met., • Consult with Physician Advisor, Psychologist Advisor and/or Clinical Leadership as clinically appropriate., • Generate denial letters based on physician reviews and in accordance with state regulations as needed., • Demonstrate an understanding of Evidence-Based Practices (EBPs) and connect members to appropriate evidence-based treatment., • Collaborate with other CBH departments to ensure quality of care, promote placement of members in the most appropriate services and to facilitate consistent longitudinal care management., • Document provider quality concerns in accordance with CBH policy/procedure., • Use data to inform clinical decision-making., • Maintain documentation in CBH’s electronic system consistent with agency and industry standards., • Respond to care coordination correspondence within 24-48 hours or sooner if necessary., • Attend all meetings as assigned and actively prepare for and participate in supervision and clinical case consultations., • Ensure the highest quality of customer service when interacting with members, providers, and other stakeholders., • Display commitment to member/family-driven care and principles of resiliency and recovery., • Contribute to projects and other duties as assigned. Contacts consist of visits made by the IHD Caseworker to meet with the youth face to face for a brief contact or transporting a youth on an activity with other youth where IHD Caseworkers conduct educational, life-skills counseling, recreational, cultural and social activities that may benefit the client., • The IHD Caseworker meets the objectives of providing sufficient supervision to ensure the youth's appearance in court, the client’s remaining arrest-free, the observation of curfews, and school and/ or work attendance., • When a client referral is received from the JJC, the first face-to-face contact must be made within 24 hours., • At the initial meeting with the youth and the family in their home, the IHD Caseworker describes the program requirements (curfews, school attendance, and making court dates). Required forms are completed and a schedule worked out for the coming week., • The IHD Caseworker makes clear to youth and family that he/she is required to report immediately to the JJC IHD Caseworker the following., • His/her arrest, • An absence without permission, • Curfew violation, • Unexcused absence from school, • Relocation without permission, • Serious family breakdown, • Missed court date, • Any serious incident that involves police, fire, or medical attention, • The IHD Caseworker must ensure court dates are kept and that hearing results are communicated to the program director., • The IHD Caseworker ensures the youth's court appearance by picking up the client and family at their home and taking them to the scheduled hearings., • The IHD Caseworker provides written court reports for court., • Upon arrival at court (usually by 9:00 a.m.) the IHD Caseworker checks the youth in with the court clerk. He/she/they are also responsible for making contact with the client’s attorney, usually a Public Defender, explaining that the client is committed to the IHD program., • The IHD Caseworker is required to turn in proper court reports, progress reports and any other reports to the court and attorneys concerning the client’s adjustment and or progress., • The IHD Caseworker accompanies the youth and family into the court room and must be prepared to make an oral report and to answer any questions the Judge may have., • If the case is continued, the IHD Caseworker must specifically ask the Judge or clerk if the youth is still committed to the In-Home Detention program., • The IHD Caseworker is responsible for explaining the court outcome to youth and family., • The IHD Caseworker is responsible for maintaining the youth’s record in accord with JJC policies and procedures. Any concerns about a family or child must be immediately communicated to the Clinical Supervisor or then the next higher administrative position whether in or out of office hours, • A therapist must accept any case assigned to him/her. Discussion around allocations will take place and there is room for negotiation, but the Clinical Supervisor makes the final decision. Once a case is assigned, the therapist has to maintain the responsibility for the case up until a point of transfer of responsibility, this point of transfer being the decision of the Clinical Supervisor. Therapy is focused on resiliency factors and connection to community-based supports as identified by the treatment team. The MT utilizes a strengths-based approach to empower children with emotional and behavioral needs to thrive and to help the child and family reach their goals. Mobile therapy services consist of individual therapy, family therapy, development and revision of the ITP (as needed), assistance with crisis stabilization and assistance with addressing problems the child, youth or young adult has encountered. The programming builds upon youths’ resiliency and sense of self-worth for positive outcomes and redirection. There are no traditional office hours. Case Managers will work primarily in the afternoon and evenings, specifically during the school year and at weekends to provide maximum involvement with the family and the identified clients. There may need to be a need to start early to help a participant get to school. The Case Manager will be placed on an on-call roster and will be responsible for providing emergency assistance to clients assigned in the program 24 hours a day while on duty. POSITION RESPONSIBILITIES:, • IPS is a program designed to provide a safe and holistic environment for at-risk youth to develop themselves and their confidence, through an engaging, supportive and strength-based environment. IPS staff will develop programs and activities that include the 7 core concepts of resilience, which are the program’s core values., • The major responsibility, therefore, is to resolve issues, ensure stability and to develop positive youth development and academic support., • The IPS Case Manager is responsible for providing service linkages for all members of the family as appropriate., • The IPS Case Manager will facilitate all academic progress in collaboration with the youth’s school., • The Case Manager will collect all academic records to determine current academic status of youth and make personal contact with school counselor and teachers to gather all necessary educational information including attendance, behavior reports and Statewide testing reports., • The Case Manager will develop an academic support action plan with the school counselor thorough n inter-agency meeting with the goal of improving and supporting the youth academic progress, • Coordinate and monitor the implementation of the academic support action plan., • Engage care givers to take a more active role in the youth’s school settings by accompanying them to school meetings and attending school functions., • Assist the parent/caregiver to advocate for their child’s educational needs and helping them navigate through the school support system and introducing them to the Educational Law Center and other educational advocacy groups and parent groups., • Coordinate with school counselor to develop post-secondary plan for older youth., • Ensure prior to and during the discharge planning the enrollment in after school or other remedial or enrichment activities as needed, especially beyond the youth’s involvement in the IPS program., • The IPS Case Manager will co-facilitate the site based program in which each youth will participate three times a week., • The main tenets of the role include identification of, and referral to, needed services, agency leadership in coordination, and active goal-directed support for the youth and family., • The Case Manager will work collaboratively with the assigned counselor and develop activities and coordinate community service/engagement programs. At crisis times, there may be a need to “double team”., • The maximum caseload is 17 clients. Case longevity is determined by court and or DHS personnel., • The Case Manager’s role is to be able to report to the court that identified issues have been resolved through a collaborative effort with school personnel and community resources and through the site-based programming activities., • Case Manager must meet with the supervisor weekly for supervision., • Case Manager must meet with the family and see all children once each week, and must coordinate work with others involved with the family, possibly including parent aides.
