Job Description Position Summary We are seeking a compassionate, organized, and motivated Case Manager to provide comprehensive case management services to young adults (ages 18-24 or 18-25) residing in a shelter setting. The Case Manager will help residents achieve stability, independence, and permanent housing by developing individualized service plans, connecting clients to community resources, and providing ongoing support in a trauma-informed, client-centered environment. The ideal candidate is passionate about working with young adults experiencing homelessness and understands the unique challenges related to housing instability, mental health, employment, education, and life skills development. Essential Responsibilities • Conduct comprehensive psychosocial and needs assessments upon client intake., • Develop and maintain individualized service plans that address housing, employment, education, healthcare, and personal goals., • Meet regularly with assigned residents to monitor progress, update goals, and provide supportive counseling., • Assist clients in securing permanent housing through housing applications, landlord engagement, and subsidy programs., • Connect clients to community resources, including:, • Mental health services, • Substance use treatment, • Medical care, • Educational and vocational programs, • Workforce development, • Public benefits, • Legal services, • Assist clients with obtaining identification documents, public assistance, Medicaid, SNAP, SSI/SSD, and other entitlement benefits., • Provide crisis intervention and conflict resolution as needed., • Maintain accurate and timely case notes, assessments, referrals, and documentation in the agency's electronic database., • Collaborate with multidisciplinary staff, including housing specialists, employment specialists, clinicians, and residential staff., • Participate in case conferences, team meetings, and supervision., • Advocate for client needs while promoting self-sufficiency and independent living skills., • Assist residents with budgeting, financial literacy, daily living skills, and community integration., • Ensure compliance with agency policies and funding source requirements., • Maintain confidentiality in accordance with HIPAA and agency standards. Qualifications • Bachelor's degree in Social Work, Psychology, Human Services, Sociology, or a related field required., • Master's degree preferred., • Minimum of one (1) year of experience providing case management, preferably in a shelter, supportive housing, youth services, or homeless services setting., • Experience working with young adults, at-risk youth, or individuals experiencing homelessness strongly preferred., • Knowledge of trauma-informed care, motivational interviewing, and harm reduction principles., • Familiarity with housing resources, public benefits, and New York City homeless services is preferred., • Strong crisis intervention, problem-solving, and communication skills., • Excellent organizational skills with the ability to manage multiple cases simultaneously., • Proficiency with Microsoft Office and electronic case management systems (HMIS, CARES, AWARDS, Foothold, or similar platforms) preferred.Company DescriptionThe Bachrach Group (TBG) is a New York-based staffing and recruiting agency founded in 1974 by Richard Bachrach.
