At DHU Healthcare CIC , our Primary Care teams work at the heart of local communities, delivering high-quality, patient-centred services across the areas we serve. Based at Eyam Surgery, with flexibility to support Bradwell Surgery when required, you'll play a vital role in ensuring patients receive timely access to the right care and a positive experience from the moment they contact or visit the practice. Our teams of skilled clinicians and support professionals work together to provide responsive, professional NHS services. As a Care Navigator, you'll be central to guiding patients to the most appropriate service, helping the practice run efficiently while always keeping patient wellbeing at the forefront. As a Care Navigator you will: • Be the welcoming, professional first point of contact for patients and visitors, both face to face and on the telephone., • Open and close Eyam and Bradwell Surgery as required, ensuring the premises are safe and ready for service., • Book appointments for patients with the clinical team and other services using the clinical system in place., • Signpost patients to the most appropriate services, including community pharmacy, social prescribing, physiotherapy and other community-based support., • Ensure messages are passed quickly and accurately to the correct member of the team via electronic systems or telephone, following surgery protocols., • Contact patients with test results and other information on behalf of clinicians., • Process repeat prescription requests and escalate queries appropriately., • Monitor and respond to incoming emails in a timely and professional manner., • Maintain tidy and welcoming reception and waiting areas, ensuring posters and information are current and appropriate., • Receive and record patient payments where applicable, balancing takings at the end of each shift., • Support daily operational tasks such as notifying the secretary of incoming post and preparing clinical rooms where required., • Adhere to all surgery policies, including Health & Safety, safeguarding and confidentiality requirements., • Provide flexible support to cover planned and unplanned staff absences across both sites. #IND1 JBRP1_UKTJ
