We have an exciting opportunity for a Physiotherapist to join St Margaret's Hospice Therapies Team. • Salary: £12,816.89 per annum (equivalent to a full time salary of £31,615), • Part time: 15 hours per week, • Contract: Permanent, • Location: Yeovil This is an excellent opportunity where your existing skills, knowledge and experience can be developed further through our ongoing investment in your professional development. You will be working as an integrated member of the multi-disciplinary team in the Hospice and in the community. Based at our Yeovil Hub, you will support patients in the outpatient, Day Centre setting and in their own homes. The service is for palliative patients at all stages of their journey from diagnosis to end stage disease, offering preventative, restorative, supportive and palliative rehabilitation and intervention. Access to own transport is required to travel with equipment across the locality during a normal working day. Be part of a welcoming, community minded charity. Responsibilities Clinical, • To be professionally and legally accountable for all aspects of own work, including the physiotherapy management of patients in your care., • To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi-pathologies using clinical reasoning skills and assessment techniques., • Focus on a Rehabilitative Palliative Care approach, to formulate and deliver an individual physiotherapy treatment programme based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills., • To identify health and social care problems requiring assessment/intervention from other agencies and with the consent of the patient action those referrals., • To formulate accurate prognosis of physiotherapy potential and recommend best course of intervention, developing comprehensive care plans in conjunction with the patient and the multi-professional team., • To assess patients understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack the capacity to consent to treatment., • To support and educate carers to empower them to understand and support patients treatment / rehabilitation programmes., • To make appropriate referrals to the multi-disciplinary team and work collaboratively with a wide range of community-based services., • To understand clinical risk within own patient case load and manage caseload with support of Specialist Physiotherapists., • To work within St Margaret's Hospice clinical guidelines and The Chartered Society of Physiotherapy code of conduct and to have a good working knowledge of national and local standards and monitor own and others quality of practice as appropriate., • If working in the community and domiciliary settings, to work as a lone practitioner, adhering to St Margaret's Hospice lone working policy and procedures. Administrative, • Planthe effectively daily management of a caseload of patients including responding to referrals and occasionally unpredictable work patterns., • To be able to prioritise clinical work and balance other patient related and professional activities in accordance with the departmental requirements., • Represent the physiotherapy service and/or individual patients at multi-disciplinary team meetings, to ensure the delivery of a coordinated multi-disciplinary approach and integrate physiotherapy input into the holistic care plan. This will include discussion of patient progress, and involvement in discharge and transfer of care planning., • To be responsible for the safe and competent use of all equipment, appliances and aids prescribed to patients., • To be able to manage potentially stressful, upsetting or emotional situations in an empathic manner., • To actively seek and identify areas where changes to routines and practices will result in improvements., • To work as a team to provide an efficient and effective service., • Actively contribute to the implementations of hospice strategic and business objectives., • To maintain and provide accurate and timely statistical information as required in consultation with senior staff., • To participate in St Margaret's Hospice appraisal process., • To ensure that all equipment defects, accidents and complaints are reported to your line manager immediately to ensure that appropriate action may be taken in line with St Margaret's Hospice Health and Safety Policy. Education, • To be responsible for and actively record your own personal development., • To keep abreast of all new developments which includes attending relevant study days and visits to specialist units in consultation with the Therapy and Day Service Lead., • To be an active member of the in-service training programme by attending staff meetings, training sessions, external courses and reflective practice., • To complete and remain up to date with all mandatory and role appropriate training on SMILE., • To be proactive with regard to learning from senior staff within the physiotherapy service., • To acknowledge and seek support with gaps in knowledge and experience, utilising both internal and external training opportunities. Research / audit, • To participate in appropriate research/evaluation projects and to be aware of current relevant research., • To participate in the planning and undertaking of outcome measures and patient and carer satisfaction surveys, contributing to action plans to address the identified outcomes., • Inline with St Margaret's Hospice commitment to clinical governance, to participate in clinical audit and evaluate your own clinical effectiveness as directed by the Therapy and Day Services Lead., • To ensure any ideas to improve quality of patient care are investigated through the physiotherapy service. Effort, • To carry out assessments and treatments of adults presenting with a range of musculoskeletal, neurological and multi-pathological presentations requiring moderate physical effort on a daily basis., • To comply with the Moving and Handling Policy and therapeutic handling guidance at all times. This job requires frequently handling of patients with significant mobility problems and can involve the use of hoists, wheelchairs, and other mobility equipment; it also involves regular fitting of equipment., • To deal sensitively with patients who have high levels of anxiety and aggression caused by pain, dementia, limited mobility, or life‑limiting illness., • To be able to transport yourself and equipment across the locality during the course of a normal working day., • To be able to spend a significant amount of time driving on a regular basis if required. Communication and people skills, • To liaise with other members of the physiotherapy team with regard to patient care, training needs and professional support with more complex patients., • To communicate effectively with other agencies/disciplines both within and external to St. Margaret's to ensure a multi-disciplinary approach to care., • To be able to communicate complex and sensitive information to patients, carers and other staff including imparting unwelcome news., • To ensure timely and effective communication with the specialist Physiotherapists and other staff on all professional matters., • To maintain accurate records in line with Chartered Society of Physiotherapy standards and St Margaret's Hospice guidelines., • To attend any meetings, or case conferences as appropriate in order to ensure appropriate and timely management of patients through to discharge., • To work effectively with those patients who first language is not English and take into consideration any diversity issues in particular differing cultural awareness., • Use a range of verbal and non-verbal communication tools to communicate effectively with patients to progress rehabilitation and treatment programmes. This will include patients who may have difficulties in understanding or communicating. For example, patients who may be dysphasic, depressed, deaf, blind or who may be unable to accept diagnosis/prognosis and to assist them to reach realism of their capabilities., • To attend and be an active participant in speciality, peer group and departmental staff meetings., • BSc (Hons) Physiotherapy or equivalent qualification., • HCPC registration., • Full UK driving licence and access to a car in order to undertake community, outreach and domiciliary work., • Member of Chartered Society of Physiotherapy or equivalent body., • Ability to adapt to changing or conflicting priorities., • Ability to demonstrate a clear understanding of palliative care and the needs of patients and families., • Ability to meet the physical demands of the role., • Ability to deal sensitively and compassionately with patients who are impacted by pain, dementia, limited mobility or life‑limiting illness., • Demonstrate a passion for Palliative Care, either through experience or a desire to learn., • Ability to communicate effectively with people about difficult or sensitive topics and/or in difficult situations., • Excellent interpersonal skills with ability to liaise effectively with patients, the multidisciplinary team and across organisations and individuals at a range of levels., • Experience in supervising support staff and facilitating their development., • Ability to demonstrate sound clinical decision making in the assessment of patients and the implementation and evaluation of treatment plans plus patient centred short and long-term goals., • To be computer literate., • Ability to demonstrate effective caseload and workload management., • Experience of multidisciplinary/multi-agency knowledge and/or working, • Ability to demonstrate postgraduate experience or study of a range of relevant topics., • Demonstrable rotational experience across a broad range of specialities., • Experience of working with Palliative Care patients., • Experience of additional techniques e.g. Acupuncture / TENS., • Knowledge and/or experience of the principles of Rehabilitative Palliative Care.
