Part Time Patient Navigator Role - 3 Days (Mon, Tues, Weds, 22.5 hours/week) The post holder will support patients through their hospital care journey, providing a single named point of contact to coordinate their care. Work as part of the Rheumatology team to ensure all administrative functions for patients—from GP referral to discharge—are coordinated according to specific care pathway instructions to facilitate a smooth patient journey. Assist patients with multiple specialist needs by coordinating their pathways to reduce the number of appointments across the Trust. Main duties of the job Support patients through their hospital care journey, acting as a single point of contact. Coordinate all administrative functions from GP referral to discharge, ensuring adherence to care pathway instructions. Help patients with complex needs navigate multiple pathways, reducing appointment burdens across departments. Supporting sub-departments within Rheumatology.
