At Mayfield Road, we provide support to adults with profound and multiple learning difficulties (PMLD) in a residential care setting. We’re committed to helping every person we support feel safe, happy, empowered enabling them to live the best life possible. * This is a nurse qualified role * What you'll be working: • Working 5 days a week, • 40 Hrs per week, • Monday to Friday, • Some on call duties over weekends. What you'll be doing: • Observe and operate at all times in accordance with the Company’s policies and procedures., • Provide a flexible working pattern, to ensure the smooth operation of service, including working out of core hours, weekends and public holidays., • To undertake any other duties appropriate and as reasonably required, to meet the changing needs of the service., • Ensuring service user needs and business objectives are met., • Ensure effective on call cover for designated services, • Responsible for keeping up-to-date with any changes, or new legislation affecting the provision of services., • Delivery of training for inductees and existing employees, as required., • Facilitate CQC inspections to ensure 100% compliant., • Responsible for ensuring the service has the required level of resources to support the service users. Including ensuring that the service is fully staffed according to agreed levels., • Maintain the resource maps, working with the scheduling department., • Monitor service provision, to ensure full compliance with all relevant legislation, in line with organisational, regulatory and legal requirements., • Support Quality Assurance activities of own and other services, achieving 100% compliance., • Acting as a representative of Active Care Group at all times., • Liaising with external stakeholders, including medical professionals, care managers, families, advocates, therapists, prospective service users., • Supporting referrals activities - including service visits and assessments., • Support service users' ongoing medical needs., • Support service users who may require behaviour management. Reporting to the Manager Health Services the Healthy Living Facilitator is part of a multidisciplinary Public Health-Healthy Living team that uses principals of population health promotion health equity and community health development to support communities and groups in areas of assessment program development and capacity building for health. Activities focus on primary and secondary prevention and health promotion with a responsibility to maintain communication with Southern Health-Sant Sud staff and community stakeholders. The Healthy Living Facilitator will work with communities as they identify develop implement and evaluate initiatives including Health Public Policy that promotes health. The incumbent will exercise the appropriate level of initiative and independent judgment in determining work priorities work methods to be employed and action to be taken on unusual matters. The position functions in a manner that is consistent with the mission vision and values; and the policies of Southern Health-Sant Sud. May be required to work in locations other than their site/office within the PHHL program. • Minimum of two (2) years experience in community health development and/or population health promotion., • Experience working with populations with diverse cultural and social backgrounds would be considered an asset., • Training and experience in mental health promotion would be considered an asset., • Proficiency in Microsoft Office applications and Outlook., • Demonstrated ability in marketing and evaluation of health promotion programs communication critical thinking and the ability to work independently and as part of a team., • Demonstrated ability in leadership using a broad range of comprehensive and multi-faceted strategies to strengthen community action re-orientate health services develop personal skills create supportive environments and build healthy public policy., • Given the cultural diversity of our region the ability to respect and promote a culturally diverse population is required., • Proficiency of both official languages is essential for target and designated bilingual positions., • Demonstrated ability to respect confidentiality including paper electronic formats and other mediums., • Good work and attendance record., • Requires a valid Class 5 drivers license an all-purpose insured vehicle and liability insurance of at least $1000000.00., • Demonstrated ability to meet the physical and mental demands of the job., • No hazardous or significantly unpleasant conditions., • May work occasionally evenings and weekends as necessary., • Will be required to travel to other regional facilities as the position duties may require.
