Job Description Position Summary: CTT is mandated to provide service to those individuals within the mental health system who present the greatest degree of severity of symptoms as evidenced by their multiple physical, psychiatric and legal conditions; extensive use of services and lack of follow through with treatment. These individuals measure at the highest level of medical necessity as defined by the state Adult Environmental Matrix. The ACT Teams are specialized teams that provide multidisciplinary clinical intervention, treatment, and assessment of individuals who may be young adults, heavy users of services, or actively involved in the criminal justice system and may also have co-occurring behavioral health diagnoses. ACT team staff provides case management supports consistent with the Community Support Program of Philadelphia. The Extended Care Vocational Specialist provides a comprehensive assessment process with the intent to offer recommendations and renders therapeutic and vocational services as required to address issues that might present a barrier to flourishing in the community. The Extended Care Vocational Specialist is also responsible for ensuring that the mission, goals and philosophy of the Community Treatment Teams (CTT) are operationalized within the team. The TIPS program will provide a comprehensive, intensive early intervention services for youth exhibiting truancy behaviors. The programming builds upon youths’ and families’ resiliency and sense of self-worth for positive outcomes and redirection, strongly networking with community resources. Assignments are based on a 40-hour week. Weekly schedules are developed based upon the caseload needs. There are no traditional office hours. Case managers will work primarily in the afternoon and evenings, specifically during the school year and at weekends to provide maximum involvement with the family and the identified clients. Essential Functions: • Travel throughout the city where drug trafficking activity is prevalent for outreach programs., • Conduct psychosocial assessments to document the history and identify preliminary issues for treatment focus through the interview process., • Attends regular treatment team meetings to provide a social work perspective to total case management of the resident and state or local agency legal requirements., • Conduct educational and other didactic groups for residents and family members., • Develop/Coordinate Individualized Discharge Plan., • Makes handwritten entries to resident charts describing therapy performed, progress in the discharge planning process, or other interventions., • Reports incidents of suspected abuse using prescribed forms and protocols from the relevant community/state agency., • Develop and deliver community education programs to enhance community awareness of mental health issues and resources available to address these issues and support the residential’s strategic marketing effort., • Responds to psychiatric emergency codes utilizing training in the management of aggressive behavior., • Participates in training and development for the purpose of professional growth and skill enhancement., • Provide backup services for incoming assessment and referral calls., • Support the residents' safety and security program. Job Requirements: • High school diploma required. Bachelor’s degree is preferred in the area of human services or sociology, political science, criminal justice, pastoral counseling, education or other related field., • Experience in an in-resident setting preferred;, • Written and oral communication skills, • Skills in case management, time management, and crisis intervention;, • Skills in maintaining information as highly confidential. Hearing is to perceive the nature of sound with no less than 40 db loss a Hz, 1000 Hz, or 2000 Hz with or without correction., • Ability to perceive detailed information through communication and make fine sound discriminations., • Individuals must be able to exert up to 100 pounds of force occasionally and to be able to lift, carry, push, pull, or otherwise move objects., • Work requires a minimum standard of visual acuity with or without correction that will enable people in the role to complete administrative and clerical tasks and visually observe patients on the unit and in therapeutic activities., • While a worker may possibly be subjected to temperature changes, the worker is generally not substantially exposed to adverse environmental conditions as the work is predominantly inside., • Required to travel throughout the city where drug trafficking activity is prevalent.
