Job Description The Clinical Care Manager is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Essential Functions: Depending upon your team assignment, some or all of these may apply. • Be knowledgeable about behavioral health care services, system partners, service continuum and established medical necessity criteria and apply that knowledge in all work activities., • Complete Utilization Management activities including but not limited to: interagency meetings, social determinants interviews with members, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage., • Complete expected number of UM activities per day/shift as specified under team specified under team specific responsibilities in established timeframes., • Authorize and extend treatment authorizations when Medical Necessity Criteria is met., • Consult with Physician Advisor, Psychologist Advisor and/or Clinical Supervisor as clinically appropriate., • Generate denial letters based on physician consults and in accordance with state regulations as needed., • Complete discharge reviews in a timely manner., • Work directly with members, natural supports and providers in the community during 30 - 75% of work hours or 6 - 15 days per month., • Facilitate care coordination with system partners to address social determinants, barriers to wellness, and other needs of members, including connection to appropriate community based services., • Demonstrate an understanding of Evidence-Based Practices (EBPs) and connect members to appropriate evidence-based treatment at admission, during treatment, and at discharge., • Monitor member’s length of treatment and work collaboratively with providers and other systems to address barriers to member receiving treatment in a less restrictive environment., • Collaborate with other CBH departments to ensure quality of care, promote placement of members in the most appropriate services and to facilitate consistent longitudinal care management., • Document provider quality concerns in accordance with CBH policy/procedure., • Conduct provider meetings at least quarterly, more frequently based on provider and supervisory guidance., • Use data to inform clinical decision-making., • Maintain documentation in CBH’s electronic system consistent with agency and industry standards., • Understand and participate in grievance/complaint hearings as assigned., • Respond to care coordination correspondence within 24 hours, or sooner if necessary., • Work collaboratively with team members including assisting with the training of newly hired employees as requested, covering work when peers are out of the office as assigned and finding coverage when absent., • Attend all meetings as assigned and actively prepare for and participate in supervision and clinical case consultation., • Maintain an accurate daily log of work completed., • Manage providers of IBHS programs., • Complete an average of 8-15 UM activities per day., • Review and process authorization requests, as assigned, including: review of documentation, Physician consultation per Review Guidelines, provider notification, referral submission (if required), insufficient or denial letter (if required), authorization generation (if required), and log of review., • Participate in at least 8 interagency meetings per month for members receiving IBHS. Job Description Position Summary: The Behavioral Health and Justice Division (BHJD) is committed to working towards reducing criminal justice involvement for Philadelphians with behavioral health challenges, including Serious Mental Illness (SMI). BHJD organizes its work according to the Sequential Intercept Model, identifying opportunities to intervene at various points of criminal justice involvement to deflect and divert individuals away from further penetration of the system and into the behavioral health system. By helping connect individuals to needed treatment and support services that promote stability in the community, BHJD believes that Philadelphians are less likely to be arrested and can achieve health, well-being, and self-determination. BHJD also aligns with Stepping Up and focuses its efforts on lowering the number of Philadelphians with SMI admitted to the Philadelphia Department of Prisons (PDP), reducing their length of stay, connecting them to treatment/supports upon release, and reducing recidivism. The Forensic Support Team (FST) of BHJD was established to meet the unique needs of Philadelphians who are either: 1. Adjudicated as not competent to stand trial on only misdemeanor charges and committed to a State hospital (e.g., Norristown State Hospital) or 2. Adjudicated as non-restorable. The FST works within PDP and Norristown State Hospital (NSH) in order to assist individuals while in jail, when transitioning from jail to NSH, and when reentering the community typically under the court’s jurisdiction. To support this forensic population, the FST helps individuals navigate the Philadelphia judicial and behavioral health systems while also seeking to address each individual’s unique legal, behavioral, medical, physical, cultural, and/or financial needs. The Clinical Assessor becomes an important part of a team comprised of: one (1) FST Supervisor (a licensed psychologist); one (1) FST Coordinator, three (3) Clinical Assessors, and one (1) Post-Doctoral Psychology Resident. The Clinical Assessor will complete clinical assessments and provide system navigation for justice-involved individuals with behavioral health challenges who fall into either of the aforementioned priority groups. In this capacity, the Clinical Assessor is responsible for determining the appropriate level of care (LOC) for the individual via a procedure that is consistent with a strengths-based, biopsychosocial recovery model. The procedure relies on State hospitals and jails to provide access to the individual as well as clear documentation and consultation about the individual so an informed decision can be made about the appropriate LOC and the individual’s risk for violence. The decision about the LOC is based on the individual’s: aspirations, goals for recovery, strengths, support system, risk for future violence, psychiatric diagnosis, medical diagnosis, psychiatric history, cognitive or physical limitations, results from any psychological measures or medical tests, current medications, incident reports, participation in recovery services, current treatment team input, and privileges earned. With this information and documentation, the Clinician Assessor collaborates with the Philadelphian and their treatment team in an effort to agree on an appropriate LOC as well as a re-entry plan and, when necessary, present the plan to the appropriate criminal court. Duties and Responsibilities: The Clinical Assessor helps individuals with SMI "navigate" the maze of treatment programs, the court system, and support services upon reentry into the community. The Clinical Assessor reduces barriers that keep individuals with SMI from accessing timely treatment by identifying behavioral health needs and directing individuals to appropriate resources. An effective Clinical Assessor is a: Relationship builder Building effective working relationships is the foundation of effective forensic navigation. To improve coordination of care for people with mental illness and to reduce barriers to reentry, navigators must work with different groups of people: individuals who are incarcerated, healthcare teams within the PDP, criminal justice partners, families, NSH staff, and resource providers within the community. It is important to build effective working relationships with all of these groups. Clinical care coordinator The Clinical Assessor conducts forensic evaluations and coordinate care for individuals who are either: 1. Adjudicated as not competent to stand trial on only misdemeanor charges and committed to a State hospital (e.g., NSH) or 2. Adjudicated as non-restorable. They coordinate with the court system, NSH, and treatment providers to determine the most appropriate and effective interventions and levels of care, and they provide the needed linkages. They plan reentry for individuals who will be returning to the community. Moreover, Clinical Assessors enhance the continuity of care as individuals transition between PDP and NSH. Creative problem solver The Clinical Assessor finds creative solutions to barriers. Some individuals with mental illness utilize commonly used services, but others have unique needs that require initiative and creative thinking to locate the appropriate resources. Organized information manager The Clinical Assessor manages and tracks a large amount of information. Assessments, referrals, legal statuses, and treatment histories are just a few things the Clinical Assessor needs to track and, by law, this information must be kept confidential and secure. It is important that the Clinical Assessor stays organized and uses a systematic approach to information management. Specific duties • Perform level of care and other-discharge related assessments for individuals adjudicated not competent to stand trial and/or non-restorable that are detained at the PDP or NSH, • Attend treatment team meetings for individuals detained at NSH to monitor progress and/or changes in clinical presentation, • Facilitate access to re-entry services and programs, • Identify individuals for diversion and share with appropriate justice partners, • Review legal status, commitment type, charges, competency opinions and findings, and clinical needs for all individuals on the waitlist to NSH on an ongoing basis, • Maintain accurate, confidential records documenting services provided and unmet needs using appropriate databases, • Remain current with resource and referral information, • Maintain list of community resources, • Build and maintain relationships with key individuals (e.g., PDP staff, Public Defender), and provide updates upon marked changes in an individual’s clinical or legal status to trigger appropriate interventions, • Coordinate with courts to ensure timely court hearings and legal proceedings, particularly when an individual’s clinical or legal status changes, • Provide behavioral health literacy and continuity of care training to both behavioral health providers and criminal justice personnel, • Provide updated progress reports for criminal court hearings and attend those hearings when necessary This position requires a master’s level degree in social work, psychology, or a related clinical focused field with at least two years of experience working with individuals with serious mental illness and/or in clinical-forensic assessment. Knowledge of and experience in working with the Philadelphia criminal justice system and/or the Philadelphia behavioral health system are preferred. The position requires demonstrated skills in communication, problem solving, decision making, and report writing. The individual possesses the ability to skillfully communicate verbally and in writing with a variety of multidisciplinary partners, including judges, attorneys, jail staff, provider agencies, psychiatrists, nurses, and social workers. Physical Demands: Ability to physically perform the duties and to work in the environmental conditions required such as maneuvering in office space – reaching file cabinets, fax and copier machines when necessary. Must be able to sit for up to 2 hours looking at a computer monitor and using a keyboard and mouse to perform data entry functions. Position Summary The Case Manager (Advocate) is responsible for providing culturally sensitive one-on-one health and parenting education. Responsible for monitoring and case-management to infants recovering from Neonatal Abstinence Syndrome (NAS) and exposed prenatally to opiates or other substances. Case management will also be provided to mothers with Substance Use Disorders or other caregiver when baby is in kinship or foster care. A High School or Associate's degree and a minimum of 12 semester credit hours in social science and 2 years experience in public or private human services with 1 year in direct client contact or a Bachelor's degree with major coursework in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education. Job Description Our Mission: Enable individual persons who have serious mental illness, co-occurring substance use disorder and/or a dual diagnosis of SMI and IDD live a life that is meaningful to them in the community on terms they define. Title: Case Manager (CM) Supervisor: Program Director Summary of Job Description: Help to develop and implement treatment plan goals and targets for Members in collaboration with the multi-disciplinary team. Participate in crisis management, program activities, and provide direct oversight for all members. The CM works closely with members on goal attainment and supports members as they transition into less structured living arrangements. Specific Responsibilities: • Collaborate with admissions to support the admission and acclimation of new Members into the program setting., • Complete Incident Reports in accordance with processes outlined by the Managed Care Organization (MCO), State and Project Transition/ PCS Mental Health Policy and Process., • Collaborate with members of the multidisciplinary team to develop treatment plans for identified Members., • Complete weekly individual Case Management sessions with Members and document in DAP format., • Facilitate/ Co-facilitate all groups as assigned and complete documentation., • Support with transporting Members to appointments per the directive of the Program Director., • Support with observation of Member drug and alcohol screenings and document observations (if applicable)., • Participate in phone coaching/ crisis management process, which includes (but is not limited to):, • Orienting Member and family to phone coaching process., • Carry and respond to coaching phone calls on days assigned, including being available to go onsite for emergencies during hours when there is no or minimal staff coverage., • Provide coverage for staff callouts., • Following incident reporting and documentation process for all on-call emergencies., • Support Members with links to services within the community., • Complete family contacts for assigned Members per requirements., • Ensure all Members have an emergency contact documented., • Ensure all consents are completed and updated yearly., • Review all Member’s Advance Directives annually., • Support Members with activities of daily living including (but not limited to) the following:, • Daily oversight and coaching of daily living activities., • Observing Member self-administration of medication., • Support with the development of socialization/ communication skills, conflict resolution, de-escalation skills, and utilization of crisis supports., • Support with meal planning, menu development, food shopping and meal preparation., • Encourage and support health and home safety practices., • Support with navigation of public transportation services., • Completion of residential roving and checklist to ensure safety., • Support with Discharge Member Follow-Up., • Complete continuing educational requirements as aligned with regulatory compliance., • Attend individual supervision sessions and program meetings as scheduled by direct supervisor., • Ensure documentation of all Member engagements is completed within 24 hours., • Participate in multidisciplinary treatment team and will support and implement interventions and directives as directed by the Team., • Always demonstrate compassion and concern when supporting a Member through embracing Project Transition/ PCS Mental Health’s Mission and Core Values., • Approach Member engagement from a non-judgmental stance understanding that a Member’s behavior is driven by experience, which may include trauma., • Treat and speak to Members with supportive kindness even when a Member demonstrates intense behavioral or emotional actions. The case manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. Major Responsibilities Team-Based Planning • Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective., • Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant., • Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.
