The DESC team has been providing mobile crisis response services to the community for well over a decade. JOB DEFINITION: Each Crisis Outreach Shift Supervisor is responsible for ensuring their team’s timely, effective, and coordinated response into the field as they are dispatched and deployed by Crisis Connections. Shift Supervisors work alongside the staff they supervise providing real-time, on the ground supervision, coaching and guidance for how to resolve crisis situations their teams are responding to in the community. Crisis Outreach Shift Supervisors have the responsibility of implementing the systems created by the MRRCT program leadership related to proper documentation standards, implementation and adherence to DESC policies and procedures, supervision and performance management of their team, and ensuring their team members participate in all relevant training and on-going education necessary. As part of the MRRCT each shift supervisor will at times be responsible for providing clinical and psychosocial assessments in the field and mental health evaluations to determine least restrictive alternatives to involuntary treatment. JOB DEFINITION: The Unit Technician works as a member of the multidisciplinary team to provide direct support to residents to assist them in maintaining safe, healthy, and stable living conditions within their units. This position blends supportive services with light maintenance and unit remediation tasks, working collaboratively with other members of the housing team in promoting long-term housing stability. MAJOR DUTIES AND RESPONSIBILITIES: • Participate in shift briefs and read logs before the start of each shift daily, • Interact professionally and respectfully with residents by maintaining confidentiality and uphold tenant rights at all times, • Engage and have frequent contact with tenants on the Unit Elevated Concern List (UECL), • Document unit conditions before and after occupancy using housing service notes and/or UECL in Chasers, • Follow all HUD, local housing authority, and fair housing guidelines, • Conduct regular inspections of units identified to ensure compliance with safety codes and housing quality standards and report/correct any safety hazards or code violations., • In collaboration with residents and their treatment team, complete the Housing Retention & Stability Plan in the UECL and help clients achieve their short and long-term goals., • Complete documentation on all client interactions according to agency standards and prepare and submit documentation of services., • In partnership with other staff members, help facilitate or lead activities that contribute to long-term housing success and improve independent living skills., • Assist with compliance inspections from housing authorities or funders., • Respond to urgent issues such as water leaks, power outages, or lockouts, • Assist with grounds & common area upkeep. Ensuring cleanliness and safety of hallways, laundry rooms, lobbies, and outdoor areas, • Perform routine low-level maintenance and repairs in residential units and common areas. Repair basic building or apartment systems as assigned, including basic electrical systems (switches, outlets, breakers, fixtures, and minor wiring), general carpentry needs (floor repair, wallboard and plaster work, painting), and basic plumbing., • Develop, design, and implement mitigation strategies to limit frequency or severity of unit damage in assigned units, including but limited to, installing features designed to limited future damage, • Respond promptly to maintenance requests and work orders, • Communicate repair or extensive cleaning needs to supervisor before starting work, seeking assistance to complete maintenance work not within personal skill level., • Assist with unit turnovers, new resident leasing and orientation; rent collection and facilities inspections, • 1 year of relevant maintenance or trades experience., • Basic understanding of homelessness and various characteristics of homeless adult population., • Ability to communicate and work effectively with staff from various backgrounds., • Ability to work effectively with clients displaying a wide range of unpleasant and/or bizarre behaviors., • Subscription to philosophy of cooperation and continuity across programs and of consideration and respect for clients., • Demonstrated ability to address basic maintenance and repair needs in a residential setting, • BA degree in social or behavioral science., • Residential property management experience., • Experience with the challenges of mental illness and substance use. Job Description "A New Approach to an Age-Old Need" - Pathway Geriatrics is a nurse practitioner-owned medical startup headquartered in Redmond, WA, currently serving the Greater Seattle area. We provide in-home primary care services for homebound older adults. Said differently, we bring our medical services to our clients, who often can't leave their homes to get the specialized care they need. We have a uniquely deep background in technology (and tech startups), which heavily influences our workflows and culture. Our vision is to build a new type of medical practice that melds the latest technologies and best practices with the caring hearts of top-tier providers. The Basics We are looking for an experienced Geriatric Nurse Practitioner to serve as a palliative primary care provider in our Seattle Eastside region. You will work with high-complexity homebound clients primarily residing in professionally managed residential facilities. The position is fully field-based, although we do have weekly team meetings at our Redmond office. Typical business hours are 9:00 AM to 5:00 PM, Monday - Friday. Compensation Compensation typically ranges from $130,000 to $160,000, depending on experience and performance. High performers also have the potential to earn meaningful additional discretionary bonuses. The Job Details You will serve as the PCP for a panel of about 200 homebound older adults in your region. Most of our clients live in Adult Family Homes (AFH), with a portion residing in assisted living facilities. We often have several clients at each home. We aim to visit each facility every five to six weeks, and clients are seen based on medical necessity. Our providers also facilitate each client's medication and chronic care management between in-person appointments. We do our best to keep clients out of the emergency room. While we don't provide in-home urgent care (yet), we can often manage urgent situations remotely with help from our local network of in-home diagnostic services. These include mobile imaging, ultrasound, phlebotomy, and urinalysis/urine cultures. Your Clients The nature of our services means we often work with multiple parties when coordinating patient care. We typically have at least three clients per patient: the patient, their medical power of attorney, and their caregiver (typically an Adult Family Homeowner). A typical client is 75+ years old and often in their final years. Most clients are homebound due to advanced dementia, but many also have other chronic diseases like osteoarthritis with contractures and pain, Parkinson's disease, heart failure, and diabetes. That said, we also work with younger adults who are homebound due to traumatic brain injuries, strokes, or mental health conditions. Our clients' family members and powers of attorney have varying levels of engagement and knowledge. We are transparent and communicative with them and aim to serve as a resource for them and their loved ones. The Office The job is 100% "work from homes." :) On a given day, you will visit clients at between one and five different homes or facilities. Each Adult Family Home is someone's house in a regular neighborhood that the state has licensed to care for (around six) homebound adults. Be prepared to pull out your laptop and perform your visits on the family couch, at the dining room table, or perched on a client's bedside. Adult Family Homes are unique settings that genuinely feel like family for these older adults, where they live in a small and compassionate environment, and their caregivers know them well. Their intimate knowledge of each client's habits and expressions often helps us fill in the gaps when clients cannot speak for themselves. We develop a rapport and work together with the caregivers since they have the knowledge that allows us to find the right fit for a client's treatment. Requirements Our practice approaches issues from a palliative care perspective. Our primary goal is to improve client comfort and quality of life while reducing unnecessary medications. We monitor disease progression and consider when they should transition to hospice services. We usually continue as the client's attending provider while they are on hospice, managing their end-of-life care. We see many routine problems day-to-day, but we are also frequently surprised by things we've never seen before. • We often work with dementia-related behavioral concerns expressed through tearfulness, restlessness, physical aggression, combativeness, and sleep difficulty., • As our clients grow weaker, they will transition from walkers to wheelchairs to hospital beds and require our assessments and orders to coordinate that equipment., • We partner with local home health care teams when our client needs skilled nursing, physical and occupational therapy, speech therapy, and psych nursing., • Adult family homes also have special state regulations, so they need us to provide specific orders and permissions for their inspection requirements. Typical Medications We Work With Many dementia behaviors are managed with psychotropic medications. As such, we need you to be comfortable weighing the risks and benefits of applying antidepressants, antipsychotics, anticonvulsants, and others to help our clients with their quality of life. We also strive to reduce or stop unnecessary medications. Standard "adult" guidelines often don't apply to a geriatric population. You will also need to be comfortable managing specialty medications, including those for cardiac issues and diabetes. Other Tidbits Personal Vehicle: You will need one. On-call: We are available to our clients 24/7 for urgent needs, including holidays, weekends, and nights. We rotate being on-call evenly between our providers, answering urgent calls and messages. It won't require physically going to a facility after hours, just triaging over the phone. For your reference, most nights, we don't have any calls. Giving Back: We try to give back to our community in many ways, and we ask all our employees to participate. CHMHA has a fantastic part-time opening for a Psychiatrist/Psychiatric ARNP. This role will provide psychiatric services in our agency through the prescribing and monitoring of medications, evaluation of psychiatric status, and consultation to non-medical team member. This is primarily a clinical position and incumbents will spend the majority of their time seeing patients, with the same productivity standards and same scheduling guidelines as physicians. RESPONSIBILITIES • Independently evaluate patients for the purpose of determining a differential, psychiatric diagnosis and biopsychosocial case formulation., • Assess patients for psychiatric/psychotropic medication necessity., • Prescribe the psychiatric or psychotropic medications, which are within his/her licensing purview., • Coordinate with program manager in assigned programs to facilitate integration of medical issues with overall treatment planning, • Screen patients for concurrent non-psychiatric medical conditions and refer for appropriate care., • Order all appropriate laboratory tests to determine a differential diagnosis and assure safe prescription of medications., • Assess for medication side effects and treat when appropriate, following guidelines set by accepted professional standards and any other applicable standards of care., • Consult with the treatment teams or other agency and non-agency providers on issues related to an individual patient's treatment and overall treatment planning. Liaison with primary care physicians and their staff., • Attend and participate in medical staff meetings, as required., • Obtain supervision, consultation or guidance by an agency psychiatrist or the medical director in cases as needed., • Maintain clinical records in accordance with agency policy., • Perform other functions, e.g. teaching, supervision of nursing staff and students, inter- and interagency committee responsibilities, as assigned. These responsibilities may include provision of group staff consultations without the presence of a staff psychiatrist., • Understand recovery concepts in behavioral health., • Apply recovery concepts, as appropriate to clinical work at CHMHA., • Other duties as assigned QUALIFICATIONS • Current First Aid/CPR certification or ability to obtain within 30 days of hire, • Experience working with people with severe and persistent mental illness, • Must hold a valid drivers license for 1 year with clean driving record in order to use agency vehicles., • Have legible handwriting, • Ability to do simple mathematics, • Basic computer skills, • Ability to read and comprehend agency/member charts
