We're looking for an Acute Clinical Liaison who wants their work to mean something — someone who brings clinical expertise, clarity, and compassion to moments where patients are preparing to continue care at home. Why This Role Matters This role plays an important part in helping LUX Infusion deliver a patient‑first experience by ensuring patients and caregivers feel informed, confident, and supported when transitioning from the hospital to home infusion therapy. In this role, you'll contribute to work that: • Helps patients safely and confidently manage infusion therapy outside of the hospital, • Supports hospitals, physicians, and care teams through seamless discharge and referral coordination, • Strengthens continuity of care during critical transition moments Everything we do connects back to real people navigating complex healthcare journeys — and this role helps turn uncertainty into understanding at a pivotal point in care. What You'll Do Responsibilities include: • Serve as the primary clinical educator in the market for infusion teaching demonstrations with patients and caregivers, • Provide in‑hospital education on home infusion therapy, including IV administration, flush procedures, pump usage and troubleshooting, • Accurately prepare, administer, and monitor pharmacological agents, as applicable, • Assess patients and their home environments for appropriateness for home infusion care, • Identify psychosocial considerations, adverse reactions, and potential complications, and coordinate appropriate interventions, • Provide patient and caregiver education on therapy indications, medication actions, interactions, side effects, and monitoring requirements, • Maintain thorough, accurate documentation of all education, teaching sessions, and patient interactions in the medical record, • Obtain necessary discharge documentation and referral information prior to patient discharge, • Partner with billing and internal teams to support verification of payment prior to discharge, as needed, • Coordinate referrals in collaboration with Acute Clinical Sales Liaisons (ACSLs), admissions teams, and nursing partners, • Validate physician or authorized prescriber orders and complete referral documentation within required systems, • Communicate with ACSLs regarding patient rehospitalizations or changes in care status, • Support admissions processes to ensure smooth patient transitions from hospital to home, • Ensure all activities comply with regulatory standards, company policies, and ethical guidelinesAdditional Responsibilities, • Support ACSLs in identifying and pursuing new business opportunities within acute care facilities, • Build and maintain strong relationships with physicians, nurses, pharmacists, case managers, discharge planners, and social work teams, • Participate in patient care rounds, conferences, and interdisciplinary discussions when teaching demand allows, • Provide in‑service education and training to healthcare professionals on home infusion services and processes, • Contribute to market growth initiatives aligned with quarterly and annual goals, • Maintain clinical expertise and active licensure requirements, as applicable, • Perform other duties as assignedWhat We're Looking For You may be a great fit if you bring: • Deep care about patient education and safe transitions of care, • Strong clinical knowledge of infusion therapies and home infusion models, • Confidence teaching patients, caregivers, and healthcare professionals across settings, • Excellent verbal and written communication skills, • Strong organizational skills and attention to detail, • Ability to assess patient readiness, environment, and safety for home care, • Comfort collaborating with hospital teams, internal partners, and sales liaisons, • Proficiency with clinical documentation systems and computer applications, • A creative, resourceful approach to problem‑solving and patient support, • A professional, positive demeanor and commitment to ethical practice You don't need to check every box — we care just as much about how you connect with patients and care teams as your specific background.
