We're looking for an experienced, compassionate, and motivated Case Manager to join our growing team. What You'll Do As an IOOV Case Manager, you'll work directly with individuals and families to identify needs, remove barriers, and connect clients with the services they need to thrive. Your responsibilities include: • Conduct client intakes, assessments, and eligibility screenings., • Develop individualized service plans and measurable goals., • Provide ongoing case management, advocacy, care coordination, and supportive services., • Connect clients to housing, healthcare, mental health services, employment, education, transportation, food assistance, financial resources, and public benefits., • Assist clients in navigating complex healthcare and social service systems., • Advocate on behalf of clients to improve access to services and remove barriers., • Maintain accurate, timely, and confidential documentation., • Complete case notes, assessments, referrals, and grant-required reporting., • Collaborate with healthcare providers, housing agencies, government partners, and community organizations., • Participate in outreach events, resource fairs, workshops, and community engagement activities., • Work closely with IOOV's Housing, Health, Anti-Violence, Youth, and Community Services teams to provide comprehensive wraparound care. What We're Looking For Required Qualifications • Bachelor's degree in Social Work, Human Services, Psychology, Public Health, Counseling, Sociology, Criminal Justice, or a related field (or equivalent experience)., • One (1) to three (3) years of experience in case management, care coordination, social services, housing navigation, behavioral health, or human services., • Knowledge of trauma-informed care, strengths-based practices, motivational interviewing, and client-centered service delivery., • Strong communication, documentation, organizational, and problem-solving skills., • Ability to manage multiple client cases while maintaining excellent documentation., • Proficiency with Microsoft Office and electronic case management systems., • Valid New York State driver's license and reliable transportation. Preferred Qualifications • Experience working with LGBTQ+ communities and communities of color., • Knowledge of HIV services, housing programs, Medicaid, behavioral health, or harm reduction., • Bilingual (Spanish or another language)., • Experience working in nonprofit or grant-funded programs.
