Principal Responsibilities • Demonstrates fidelity to the agency’s service values and organizational imperatives., • Provides aftercare support to cases transitioning to trial discharge, using MST-FIT model of treatment/ therapy., • As necessary, provides additional support to cases transitioning from trial discharge MST-FIT to return to placement, • Provides crisis intervention and conflict resolution to youths and their families throughout the week, to include evenings/weekends as required but minimally on a consistent rotation schedule., • Conducts virtual and in person sessions to resident youth/families., • Responsible for, at least, weekly documentation to include case, collateral and other contacts w/I 48 hours of occurrence., • Ensures on-going support and collaboration across MST and ITM program/ staff on behalf of residents., • Prepares and submits MST-FIT engagement court reports for scheduled permanency hearings., • Prepares for and attends weekly and other ITM learning community meetings and trainings., • Attends monthly/ mandatory trainings to include 7-day ITM, 5-day MST-FIT booster trainings., • Responsible to prepare for and attend weekly supervision and MST-FIT group consultations as scheduled., • Must prepare for and attend monthly MST-FIT psychiatric consultation as scheduled., • Supports the social service department with regard to resident transition by way of MST-FIT model of treatment engagement Required Qualifications and Experience • Minimum BSW or related degree required with 2-years of professional experience and some knowledge of child welfare working with at risk youth., • Valid NYS Driver's License, • Professional, mature and dependable with a bias for action., • Ability to assess and engage youth and families is critical to the position., • Physical ability to travel throughout the community, securing resources and services, while adhering to the MST 120-day targeted length of treatment model., • Willingness to adjust schedule as needed to ensure service needs are met. Preferred Qualifications • MSW degree with licensure preferred with a minimum of 1- year experience., • Former case management experience is a plus. Travel Requirements This position offers a full-time, hybrid/in field schedule and is based in Rockaway and does require home visits and travel to other locations. Job Summary The Medicaid Managed Care Liaison (MMCL) organizes the exchange of health information, treatment planning, discharge planning, and documentation for cases assigned to him/her. This includes arranging for medical escorts, transportation and supervision to health care appointments in the community. This position maintains all clerical and clinical documentation in compliance with NY State and City regulations. Major Responsibilities/Activities for this position include: • Coordinating with the Managed Care Organizations (MCOs):, • MCO enrollment, disenrollment, transitions, • Completing transmittal forms and troubleshooting insurance issues, • Managing insurance cards for all clients., • Assisting with Primary Care Provider changes, • Seeking MCO approval for services ordered by court, • MCO credentialing and recredentialing for the clinic and providers as needed., • Facilitation of a single case agreement when a child is placed outside of the MCO service area or provider network, • Billing support for Article 29i and CFTSS services, • Ensure claims are submitted correctly, • Address billing errors as they may arise., • Invoicing and reconciliation as needed., • Administrative support of clinic functions, • Document pertinent information in the EHR, • Communicate with clients/ patients in regards to all insurance related issues., • Associate’s degree or higher required., • Knowledge of Medicaid billing processes, • Proficient in Microsoft Office, • Minimum of two years of work experience in billing and dealing with medical/ mental health claims., • Strong organizational, verbal and written communication skills, • Ability to multitask, • Compliance with:, • Required background checks, including: NYS Statewide Central Register of Child Abuse/Maltreatment, NYS Sex Offender Registry, NYS Division of Criminal Justice Services fingerprinting, NYS Justice Center Staff Exclusion List, NYS OMIG Medicaid Exclusion List., • Mandated reporter training., • Applicable provisions of the NYS Education Law with respect to hiring and retaining of licensed professionals.
