Serving as the Primary Care Manager (PCM), the provider will be the central point of contact for patients healthcare needs ensuring continuity of care, promoting preventative health, and maintaining the medical readiness of the patient population. The PCM must also be knowledgeable in military-specic healthcare requirements, policies, and procedures. Direct Patient Care: Provide comprehensive medical evaluations, including history and physical examinations, for patients of all ages. Diagnose and treat acute and chronic illnesses, injuries, and other medical conditions. Order and interpret diagnostic tests, such as laboratory studies, X-rays, and other imaging procedures. Develop and implement individualized treatment plans based on patient needs and evidence-based guidelines. Prescribe medications and other therapies, adhering to the Air Force Formulary and established protocols. Perform minor surgical procedures (e.g., wound care, skin biopsies, joint injections) as appropriate and within scope of practice. Provide womens health services, including routine gynecological exams and family planning counseling (if within scope of practice). Provide pediatric care, including well-child exams, immunizations, and treatment of common childhood illnesses (if within scope of practice). Address behavioral health concerns and provide referrals to mental health professionals as needed. Coordinate patient care with specialist and other healthcare providers. Readiness & Military Specic Responsibilities. The provider will support the Air Force mission by maintaining the health and deploy ability of service members through the following duties: Medical Readiness Oversight: Monitor and manage Individual Medical Readiness (IMR) metrics, including immunizations, Preventative Health Assessments (PHA), Mental Health Assessments (MHA), and dental readiness. Health Assessments & Deployment Preparation: Conduct PHAs, pre-deployment screenings, and other readiness evaluations. Deployment Health Management: Diagnose, treat, and follow up on deployment-related conditions and injuries. Operational Medical Support: Provide clinical support during base exercises, contingency operations, and public health emergencies as needed. Medical Review Participation: Contribute to Medical Evaluation Boards (MEBs), Physical Evaluation Boards (PEBs), and related administrative processes. Standards Compliance: Adhere to all Air Force medical standards, clinical protocols, and documentation requirements. Medical Travel: When a patients condition requires medical travel, initiate and process all required documentation in accordance with Air Force and DHA policy to ensure timely coordination of care. Fitness & Performance Guidance: Maintain knowledge of Air Force physical tness standards and waiver policies, and counsel members on strategies to meet or maintain those requirements. Command Consultation: Serve as a trusted medical advisor to unit commanders on matters aecting the health, readiness, and operational capability of their personnel. Documentation and Administration: Maintain accurate and complete patient medical records in the Military Health System (MHS) GENESIS electronic Health Record (EHR). Document all patient encounters, including diagnoses, treatment plans, and follow-up instructions, in the EHR. Complete all necessary administrative tasks, such as coding and billing documentation. Participate in quality improvement activities and peer review. Comply with all applicable Air Force regulations, policies, and procedures. Ensure accurate and timely completion of medical documentation. Teamwork and Leadership: Mentor medical technicians and other support sta. Collaborate with other healthcare professionals, including nurses, medical assistants, and specialists, to provide coordinated patient care. Participate in team meetings and case conferences. Serve as a resource for other healthcare providers on medical topics. Communicate eectively with patients, families, and other members on the healthcare team. Participate in clinic performance improvement and process optimization initiatives. Additional Responsibilities: Participate in Continuing Medical Education (CME) activities to maintain professional competence. Serve on committees as assigned. Assist with training and mentoring of medical students, residents, or other healthcare professionals. The ORCA Center builds upon DESC’s successful and innovative clinic- and field-based outpatient OUD treatment team and will collaborate closely with this team. Job Overview: This role will provide direct clinical care within the DESC Opioid Recovery and Care Access (ORCA) Center. The ORCA Center is open 24 hours per day, 7 days per week, including holidays. It offers four overlapping types of services, delivered by a team of prescribers, nurses, medical assistants, peer specialists and milieu specialists, with operational leadership support: • Care to individuals in stable condition who have experienced an opioid overdose, brought to the ORCA Center by first responders or transferred from local emergency departments., • Rapid initiation of medications for opioid use disorder (MOUD), including buprenorphine (sublingual and long-acting injectable) and methadone (under the “72-hour rule”, or Code of Federal regulations Title 21 §1306.07 (b)), for people with opioid use disorder (OUD), regardless of whether an opioid overdose has recently occurred., • Harm reduction counseling and OUD-related physical and behavioral health services, such as hepatitis C testing and treatment and HIV pre-exposure prophylaxis. PRIMARY RESPONSIBILITIES: Clinical Care • Provide evidence-based treatment for OUD, including initiation and management of buprenorphine (sublingual and long-acting injectable) and, according to DESC protocols, methadone initiation., • Conduct harm reduction counseling and provide services related to infectious diseases such as syphilis, hepatitis C, and HIV (including PrEP)., • Manage and treat conditions commonly seen in individuals with OUD, such as skin and soft tissue infections and linkage to mental health care., • Perform initial health assessments and follow-up visits for individuals starting or continuing MOUD., • Educate patients on health-related topics, treatment plans, and harm reduction strategies., • Collaborate with an interdisciplinary team of clinicians, peer specialists, and social workers to provide holistic care., • Stay updated on best practices in addiction medicine and harm reduction through continuing education., • Fully licensed as a Physician, Nurse Practitioner (Family or Adult NP), or Physician Assistant in the State of Washington., • Registration with the Drug Enforcement Agency., • Experience or training in addiction medicine and harm reduction., • Strong commitment to serving individuals with complex needs, including homelessness and co-occurring behavioral health conditions., • Familiarity with trauma-informed care, harm reduction principles, and equity frameworks., • Ability to remain open and receptive to constructive feedback., • Strong ability to approach those with differing opinions with curiosity & active listening., • Ability to work collaboratively in a multidisciplinary team environment.
