Job Description Position Summary: CTT is mandated to provide service to those individuals within the mental health system who present the greatest degree of severity of symptoms as evidenced by their multiple physical, psychiatric and legal conditions; extensive use of services and lack of follow through with treatment. These individuals measure at the highest level of medical necessity as defined by the state Adult Environmental Matrix. The Teams that provide multidisciplinary clinical review and assessment of individuals who may be young adults, heavy users of service or actively involved in the criminal justice system and may also have co-occurring behavioral health diagnoses are generic and specialized. The CM Specialist-2 provides case management supports based on the Community Support Program of Philadelphia with assisting the individual in developing skills required to enhance his/her comfort and abilities to function as a productive member of the community. The CM Specialist-2 takes the lead to act as a liaison and to coordinate team services in one of the (3) areas of their assigned specialty: housing, forensic or benefits and works in collaboration with the Individualized Treatment Team to identify goals, develop, implement and monitor the service plan. Morris Home, the only residential recovery program in the country to offer comprehensive services specifically for the transgender and gender-expansive community, provides a safe, recovery-oriented environment in which people exist as their authentic selves and are treated with respect and dignity. Position Summary The Clinician will design and direct the implementation of individualized behavioral/therapeutic treatment plans, goals, and methods of intervention made in conjunction with clinical, direct support, management staff and family or other support networks for the program participant. Provides direct clinical care services through individual counseling sessions, community and residence direct interactions as well as through support network communication. A commitment to the RHD values should be demonstrated as job duties are performed. Essential Duties and Functions Direct Care: • Completes all assessments and documentation as required by RHD and others to support current and ongoing services., • Multi-Disciplinary Teamwork, • Acts as part of a multidisciplinary team designed to provide services from a holistic, trauma informed, clinical and values-based perspective., • Leads the team of multi-discipline professionals in the development and recertification of treatment plans, ISPs, goal setting, and overall treatment objectives for each client on case load., • Responsible for organizing the input from therapists, case managers, funder assigned advocate, program specialists, behavior specialists, direct support staff, program management, medical assessments, psychological assessments and any other input into a treatment plan with measurable goals, objectives and outcomes. Requirements Qualifications • Master's Degree, • Clinical Mental Health license (LPC, LCSW, LISW), • Two years' experience required, • Experience with gender affirming/transgender individuals is preferred.
