Job Title: Registered Nurse/Clinical Supervisor Department: Nursing/Back Office Reports to: Director of Nursing General Summary: Provide health care interventions including counseling, education, and health maintenance/promotion. Supervise back office staff with Director of Nursing and Back Office Lead personnel. Review, update, create, and implement MCCN Policies & Procedures. Assist. Assist providers with patient assessments; develop care plan for each case-managed patient; provide health care interventions including counseling, education, and health promotion; dispensing medication/s as prescribed. Implement and run report of the tracking follow-up; and provide referrals as necessary. Coordinate, provide and maintain the highest standards of patient care. Manage support staff, by overseeing patient care and ensuring adherence to established policies and procedures. The Clinical RN Supervisor is charged with assigning staff and monitoring their patient care activities with appropriate, timely documentation in MDR, and assist RNs with staff evaluation, recruitment, retention and training. Qualifications: A. Education, Training, and Experience: 1. Graduate of accredited School of Nursing, with valid California License. B.S., MPH, PHN preferred., 2. Two years clinical experience as a Registered Nurse, with a minimum of one year of experience in the clinical area assigned., 3. Experience as a Registered Nurse within a United States Licensing Jurisdiction., 4. Bilingual in Spanish is preferred., 5. Experience in case management and triage or sufficient nursing experience as deemed necessary by the Director of Nursing. B. Special Skills and Special Abilities: 1. Charismatic, with ability to elicit trust, while performing counseling., 2. Detail-oriented, with ability to keep, run logs and data., 3. Excellent computer skills with ability to perform, maintain, and update electronic medical records charting., 4. Ability to work as a team member with providers, medical assistants, LVNs/RNs, clerical and other staff., 5. Ability to communicate effectively and compassionately with patients, and patient's families., 6. Ability to communicate efficiently with outside agencies to form a network of services for the patient., 7. Ability to implement and create tracking, informational and /or educational systems as necessary., 8. Ability to learn new skills, as necessary., 9. Flexibility and ability to change., 10. Culturally sensitive to needs of MCCN patient. and their family., 11. Demonstrate the ability to manage a team and proactively resolve concerns., 12. Ability to ensure smooth nursing operation at our MCCN clinics through day-to-day management and long term planning. II. Duties and Responsibilities: A. Assessment: 1. Assist provider with patient risk assessment as necessary., 2. Provide assessments to high-risk prenatal patients according to program criteria., 3. Develop care plans and intervention, with ability to track progress., 4. Triage patients as needed., 5. Assist the D.O.N schedule Medical Assistants, clerks, RN(s) and LVN(s) shifts., 6. Assign Nurses or Medical Assistants to Providers., 7. Ensure nursing operational standards are met., 8. Provide reports on productivity, quality and customer service metrics., 9. Ensure all Nursing/Back Office staff follows policies and procedures., 10. Train new team members., 11. Evaluate Nursing/Back Office performance., 12. Inform patient and family members about medical procedures and reinforce doctors/providers instructions., 13. Handle complaints or other issues., 14. Establish a compassionate environment by providing psychological support to patients, family and staff., 15. Keep records of Nursing/Back Office files, patients’ cases and procedures. B. Pharmacy: 1. Process and dispend medication., 2. Educate patients and/or family member on use of medication, its actions and side effects., 3. Perform medication inventory control. C. Counseling: 1. Provide counseling with patients and families, as needed., 2. Provide appropriate information and education with supporting research based literature., 3. Provide appropriate referrals as necessary., 4. Perform accurate and timely documentation of patient interactions. D. Follow-up and Tracking: 1. Implement and develop a method of tracking patient care and services., 2. Follow-up and document in EMR patient progress and/or non-compliance., 3. Provide appropriate follow-up sessions with documentation., 4. Interface with appropriate agencies., 5. Interact with providers, as necessary, to ensure quality patient care as necessary., 6. Implement use of computer tracking system, as needed. E. Referrals: 1. Update referral list as necessary., 2. Contact potential referral sources and establish community network., 3. Attend community meetings and seminars as necessary, and disseminate or circulate appropriate information to staff. F. Literature: 1. Organize and/or develop appropriate literature to address patient needs to be distributed, in a form of packets, etc., 2. Oversee the ordering of appropriate literature and educational materials., 3. Research on best practice articles and journals for improvement in the back office flow. G. Classes: 1. Provide health education classes as necessary; on or off site., 2. Develop curricula and oversee the development of curricula by Bachelors level Health Educator., 3. Assist the Director of Nursing / CMO with the staff development, Train the Trainer and in-service/workshops. H. Supervision: 1. Supervise appropriate staff, including orientation, training, and on going evaluation of competencies., 2. Assist Director of Nursing/Operations Manager with Providers and staff scheduling., 3. Perform scheduled /Performance review and Promotion or Exit interviews., 4. Plan, direct, and evaluate total nursing care, utilizing the Nursing Process and in accordance with established philosophy. I. Other: 1. Perform other duties deemed necessary by Director of Nursing, Medical Director, CEO and/or Operations Manager. III. Internal and External Communication: A. Internal: 1. Interact with patients and families in an appropriate, professional and caring manner., 2. Interact with peers, staff, and various departments in an appropriate, collaborating and team approach manner. B. External: 1. Interact with outside agencies in a professional, effective and appropriate manner., 2. Present oneself and Mission City Clinic in a professional manner. IV. Fiscal Responsibility: A. General: 1. Facilitate cost containment by efficient use of time, staff resources, equipment, supplies, etc., 2. Assists with scheduled on site., 3. Complete payroll forms with timely submission. V. Working Conditions: A. Environment: 1. Maintains a physically safe and clean environment., 2. Reports unsafe conditions to supervisor.
