The Mental Health Professional will develop, implement, and document individual progress for individuals with behavioral health and forensic issues who are residents of Elwyn's Young Adult Residential Treatment Facility (YA-RTF). Provides supervision and programmatic decision-making. Completes mental status exams utilizing current knowledge of the Diagnostic and Statistical Manual of Mental Disorders, version IV (DSM-IV). Provides supportive counseling and acts as an advocate to represent the individual's needs and rights with all outside support service agencies, families, etc. Assists and supports individuals in all activities of daily living. Designs and facilitates therapeutic groups. Plans and implements community integration plans. DUTIES AND RESPONSIBILITIES: • Develop and implement individual programs for individuals with behavioral health challenges and provide other individualized case management functions as directed by supervisor or director, • Provide supervision and program implementation, • Provide all appropriate documentation set forth by director and regulations including monthly summaries and individual behavioral/psychiatric assessments, • Provide mental status exam using the current DSM, • Provide supportive counseling to individuals with personal, behavioral, or social challenges, • Develop and ensure individual's adherence to behavior management plans, • Advocate for individuals with outside support agencies, • Interface with families and other interested parties within the limits of signed consents, • Design and facilitate therapeutic groups according to individual need, • Plan and implement community integration plans for individuals, • Maintain communication with medical physician, psychiatrist, and pharmacist as needed, • Maintain communication with appropriate ancillary service providers, • Assist and train consumers in all aspects of self- care to ensure a maximum level of independence for that individual. You will: * Client safety is Epiphany’s top priority, MHT’s performs this by maintaining a safe therapeutic environment for clients at all times. * Maintain daily/weekly structure and schedule for clients as outlined in client schedule. * Observation of client medication that is considered a controlled substance * Ensure clients are filling out self-observation sheets for medication. * Completing rounds at the beginning, throughout and at the end of shift. When clients are placed on Q30 rounds, completing them accordingly with appropriate documentation * Overnight is responsible for hourly rounds unless otherwise specified for high-risk clients, all rounds documented in MHT chat * Transporting clients to and from appointments, clinical services, employment if applicable, and other outside activities. Ensuring Epiphany’s driving policy is being followed and if an accident occurs, accident policy is followed * Performing Urinalysis as assigned, reading urinalysis results on a 12-panel, and reporting and recording appropriate documentation as well as communicating to appropriate supervisor. * Performing breathalyzers on clients as assigned. Reporting any results over 0.00 to on call supervisor * Reporting and completing shift notes for each shift * Following chain of command at all times. Being aware of when and how to report any, and all issues on shift, including but not limited to facilities, clients and other staff * Building positive relationships with clients and staff * Utilizing group chat when on shift for communication with other staff and supervisor * Flexibility - scheduling (coverage can be needed at any time).
