Job Description Mindful Care is hiring Licensed Master Social Workers (LMSWs) to join our West Hempstead office as Clinical Intake Specialists, supporting patients at the start of their mental health care journey. This role is designed for clinicians who enjoy assessment work, building rapport quickly, and helping patients understand the right path forward for their care... all within a structured, collaborative environment. You will play a key role in evaluating patient needs and guiding them into appropriate services, with strong support from a multidisciplinary clinical team. What to Expect • Hybrid schedule with a mix of 2 in-office and 3 work-from-home days, • Consistent Monday through Friday schedule, 8:00 AM to 5:00 PM, • Structured workflow focused on intake evaluations and patient assessments, • Collaboration with therapy and psychiatry teams to support continuity of care What You Will Do • Conduct initial mental health evaluations (90791) for patients seeking services, • Complete comprehensive biopsychosocial assessments, • Utilize screening tools such as PHQ-9 and GAD-7, • Develop and recommend appropriate treatment plans based on patient needs, • Educate patients on available services and next steps in their care, • Collaborate with clinical team members to ensure coordinated care, • Participate in clinical meetings, supervision, and team trainings, • Provide coverage support to the therapy team as needed, • Maintain compliance with HIPAA and all clinical and ethical guidelines, • Master's degree in Social Work and active LMSW license in New York, • Strong assessment and clinical evaluation skills, including mental status exams, • Experience using PHQ-9 and GAD-7 screening tools, • Knowledge of DSM-5, • Concurrent documentation skills, • Strong communication and patient engagement skills, • Ability to stay organized and manage a structured workflow, • Ability to work both independently and as part of a team, • Experience working with diverse populations, ages 12 through adulthood, • Full-time, W2 Exempt, • Monday through Friday, 8:00 AM to 5:00 PM, • Hybrid schedule: 2 days in office, 3 days work from home Why Mindful Care • Hybrid schedule with equipment provided, • Minimal administrative burden with billing and scheduling support, • No case management responsibilities, • Clinical supervision provided to support licensure growth, • Comprehensive benefits package including medical, dental, vision, 401k with match, FTO, paid holidays, and parental leave, • Opportunities for growth as the organization continues to expand If you enjoy assessment work, collaboration, and helping patients take the first step in their care journey, this could be a strong fit.
