POSITION OVERVIEW Under the supervision of the Promesa Community Residence Assistant Director and Shift Supervisor, the Community Residence Aide (CRA) has the responsibility of providing residents with the highest quality care within a therapeutic environment. The CRA is responsible for a wide range of daily living and supportive tasks. This individual must demonstrate the knowledge and skills needed to provide appropriate care based on the assigned unit. The Childcare Worker is responsible for the supervision and management of all assigned children. What You'll Do • Ensure a learning environment that helps the children develop physically, emotionally, socially, cognitively, and creatively., • Support the implementation of activities that will promote social, emotional, cognitive, physical and creative development of children., • Maintain program compliance with oversight agencies, such as DOE and DOHMH, and overall CHLDC-UCC policies., • Share job related knowledge with team members so as to promote and support their professional growth., • Participate in regularly scheduled staff & team meetings, classroom development workshops and trainings conducted by funding/leading expert agencies (on and off site) as director by supervisor and as schedule allows., • Engages in center-related services/activities such as end of year celebrations, special performances, family events, etc., • Establish and maintain positive relationships with parents/families via open communications, training and other developmental activities., • Adhere to Agency (UCDCC) policies and procedures; seeks guidance/clarification from Educational Director re: interpretation, applicability and implementation., • Strictly adhere to, and enforce, the Agency policy regarding Confidentiality and HIPAA requirements., • Demonstrates commitment to leadership development, community building, and community organizing as strategies for social change., • Other related duties as assigned, consistent with the above. POSITION OVERVIEW The Family Nurse Practitioner engages in the professional, organizational, and administrative activities to provide quality care to clients based upon their medical and psycho-social needs. The Family Nurse Practitioner along with the medical director, can develop, implement, and evaluate the medical practice delivery model in accordance with specific goals. The responsibilities of the Family Nurse Practitioner are leadership, direction and coordination in the provision of medical care and physician services supervised by the medical director and program director. The Family Nurse Practitioner assumes responsibility for applicable Federal, State and City regulations for the department. KEY ESSENTIAL FUNCTIONS • Participating in planning, development, and organization of medical services provided., • Act as health coordinator to assure the provision of education, risk reduction, counseling and referral services to all patients regarding pregnancy, HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and all other communicable diseases. Also, training and education will be provided to all staff., • Meets with all scheduled patients efficiently and reviews scheduling with supervisor daily., • Reviews all medical policies related to patient care., • Conduct physical examinations of patients upon their admission and thereafter on an annual, regular and on an “as-needed-basis.”, • Review the completion and appropriateness of all intake documentation, including patient health history (i.e., other medications and allergies), drug abuse history and previous treatment failures., • Review and approval of initial needs assessment and patient initial treatment plans and reviews., • Participates in preparation for regulatory agency site visits., • Knowledge of Federal, State and City regulatory requirements and ensures all aspects of clinical care conforms to these requirements., • Order, evaluate, adjust and terminate prescription of medication dosages using sound medical, ethical and responsible discretion and meets with the patient to discuss any adjustments in medication dosage., • Reviews all accident/incidents related to patient care and provides input into actions necessary to resolve identified problems. Implements corrective actions as necessary and appropriate., • Ensures physical participation in the Interdisciplinary Care Planning process., • Prescription and provision of treatment in accordance to patient’s medical and mental condition and only upon their individual needs., • Determine physiological opiate dependency upon admission and patient eligibility for admission to program., • Participate in a weekly and on an “as-needed-basis” case conferencing sessions with MDT members to discuss preventative treatment measure to adverse changes as well as progress in patient’s medical condition., • Assures medical staff compliance with all regulatory and licensure guidelines., • Assures medical staff competence with all applicable policies and procedures of the facility and initiates action in cases of non-compliance., • Participating in the comprehensive care planning, discharge planning and client and staff development by providing ongoing medical training and education to staff. Assume health coordinator role., • Providing medical assessment, consultation, treatment, and referrals that facilitate comprehensive and coordinated patient care., • Providing crisis intervention and consultation on medical/psychiatric emergencies., • Assessing client’s need for hospitalization and consultation with client’s Physician to include other care providers the patient is linked with and/or referred to., • Reviewing all patient medication and, if indicated, prescribes medication in consultation with Physician upon patient need., • Providing the Program Director with reports on recommendations concerning the medical services., • Provide general nursing services within scope of license and patient safety goals., • Complete medical records in accordance to organizational policy, • Perform additional relevant duties as requested by supervisor/management. POSITION OVERVIEW The Psychiatric Nurse Practitioner (NP) is a licensed independent practitioner that serves as a primary and/or specialty care provider. The NP works autonomously and/or in coordination with other healthcare professionals. The NP is responsible for facilitating advanced primary and specialty nursing services to the health center’s patients within the scope of their licensure. They possess the training and competency required while using national standards of care as a framework for managing patient care and acts as advocate for policies and practice that benefit the patient/community. KEY ESSENTIAL FUNCTIONS • Conduct psychiatric evaluations and assessments., • Conduct mental status examinations and pharmacological evaluations., • Prescribe medications, teach clients about side effects and monitor response to prescribed medications, including ordering of laboratory tests as needed., • Administer injectable medications as needed., • Complete all documentation (i.e. progress notes and psychiatric assessments) as required in a timely manner in the electronic medical record (EMR)., • Review and provide recommendations for treatment plans., • Provide expedited services to clients based on risk assessment status as clinically indicated., • Provide crisis intervention services to clients as needed., • Communicate with other medical practitioners in order to coordinate treatment for clients., • Speak with managed care insurance representatives to obtain authorizations for prescribed medications when needed., • Participate in staff meetings and case conferences., • Collaborate with primary therapists, clinical supervisor and program director to discuss status of client progress in treatment and provide input/guidance on treatment planning and risk management issues., • Maintain an 80% show rate for clients who are scheduled for regular appointments., • Provide a behavioral risk screening for new patients and share the findings with the primary care team., • Provide a comprehensive psychiatric evaluation as requested., • Provide ongoing psychopharmacological management as clinically appropriate., • Recommend and provide behavior management interventions as appropriate and functions as a member of behavior rapid response team., • Collaborate psychiatric management with the members of the primary care team and the attending psychiatrist., • Participate in inter-departmental collaborative committees as needed and quality assurance and performance improvement activities., • Document clinical activities in the medical record and maintain documentation of administrative activities., • Maintain a collaborative relation with an attending psychiatrist for all clinical activities and seek supervision as needed.
