WHAT IS A CASE MANAGER? As part of the ICMS team, the Case Manager provides field-based, direct client services to program participants. Essential Functions • Meet with each participant at least once a week or as needed to: assess, support, motivate and encourage participant to work towards their goals and/or needs and provide ongoing support by practicing a “whatever it takes” approach and Trauma Informed care, Harm Reduction and Housing First Models., • Complete initial psychosocial/mental health assessment; develop an individual service plan (ISP) and monthly action plan with each assigned participant, to establish and track client goals, including action steps and timeframe for completion of each step leading to securing permanent housing., • Strengthen job readiness skills through coaching in job search, applications, resume writing, interviewing and follow-up skills., • Assist each client in developing a personal budget, including establishing or increasing their income and/or creating a savings plan. If applicable to program, maintain accurate records of client income and/or savings., • Complete VISPDAT and/or Next Step Tool with all eligible clients to ensure they are connected with the CES system and collaborate with the SPA 7 Housing Navigator/Matcher to link clients to potential housing resources using the CES matching system., • Regularly and accurately maintain client data in HMIS/Clarity and paper file, including complete documentation/case notes of all activities, services, and outcomes achieved. Job Description Well known non-profit agency serving the homeless population is looking for an Outreach Case Manager Summary: Under the supervision of the program supervisor and program manager and as part of a multidisciplinary team, the Outreach Case Manager will perform professional case management services in connection with outreach to and linkage for homeless consumers and assist consumers and their significant figures in understanding and developing solutions to problems that lead to and result from homelessness. Receive technical guidance, consultation, and supervision to enhance skills in developing case management techniques for helping homeless clients. Utilize evidence-based case management methods in assessing clients’ immediate needs, their housing situation and needs, and the creation of treatment plans to help to ameliorate issues that contribute to their homelessness. Ability to coordinate daily work in the field to service clients, complete documentation, attend meetings, and participate in training. Coordinate treatment with the multidisciplinary team in the field as part of a homeless outreach effort in Service Planning Area 6. Essential Duties: ● Conduct immediate needs assessments with adult consumers to obtain information to complete a comprehensive treatment plan ● Working knowledge of how to present within a Multi-Disciplinary Team (MDT) due to collaboration involved with internal and external entities ● Must be comfortable working and providing assistance to homeless clients in the field. ● Establish client treatment plans that correlate with the clinical assessments of therapists, nurses and substance use counselors. ● Assist the consumer to resolve or cope with issues that contribute to mental, emotional, medical, and/or substance-related problems. ● Be familiar with and able to document in accordance with the guidelines established for the E6 program, including entering all notes, treatment plans, and outcomes into HMIS. ● Provide services primarily to clients who are living in or on LA County property, such as metro rail stations, metro bus stops, and other metro facilities. ● Provide services to homeless consumers by providing case management (including providing solutions to immediate needs, linkages to benefits, housing supports, other mental health, including physical health programs, substance use programs, food support, hygiene support, and linkages to employment or school resources. ● Ability to work with homeless consumers, adults with co-occurring substance abuse issues and mental/physical/developmental disabilities. ● Ability to document effectively in accordance with established standards of care on a daily basis. ● Participate in all required meetings, trainings, collaborations, and multi-disciplinary team meetings within the program and be prepared to discuss, educate, present or evaluate the nature and progress of presenting issues. ● Attend other meetings, training sessions, seminars, as needed to effectively perform duties. ● Coordinate program activities to meet contractual agreements, laws, regulations and funding directly with the Program Manager. ● Assume other tasks as assigned and required for this position assigned by the program manager and program supervisor. ● Required to conduct all interventions and treatments in the field. ● Responsible for coordinating transportation and transporting E6 staff and participants using the Agency van as needed. ● Provide and maintain documentation of homeless status to intake workers and to clients and keep copies in client files. ● Provide HIV/AIDS information, education and linkages to testing, housing and services. ● Knowledgeable about HIV/AIDS services and testing locations. ● Enter all outreach activity into HMIS system regularly and be responsible for ensuring the accurate entry of all team members. ● Assist with collecting and inputting information for the preparing of all LAHSA-required reports, including but not limited to QPR and APR. ● Build rapport with individuals and staff of various community organizations that serve homeless individuals, families, low-income households, and other HOPICS target populations. ● Inform, educate, and refer homeless individuals and families about the agency’s programs and services available to them. ● Maintain appropriate boundaries and adhere to the agency’s Code of Ethics and Core Values. ● Ensure that all client records/documentation are submitted. Skills, Experience, Education and Abilities Required: ● 2+ years of experience working with homeless individuals with co-occurring issues (mental/health/ disabilities/substances). ● Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds. ● Ability to work with homeless clients in the field. ● Ability to communicate effectively both written and orally with collaborative entities. ● Attention to detail, punctuality, neat and professional demeanor. ● Microsoft Word, Excel, HMIS, Internet. ● Ability to report (verbally or in writing) in accordance with the law all legal issues that surface (i.e., Tarasoff, child abuse, vulnerable adult abuse, danger to yourself or others, grave disability, etc.) ● Experience working within a multidisciplinary team. ● Knowledge of evidence-based practices and the ability to use them in the field. ● Ability to work evenings, overtime, weekends, or holidays. Environmental Conditions (Working Conditions) ● Responsible for working in any and all environments, including homeless encampments, in all areas of Service Planning Area 6. ● Extensive interaction with homeless individuals, individuals with severe physical and mental disabilities, the possibility of individuals with infectious diseases, stray animals, moving vehicles, and other possible dangers associated with working in areas of high crime. ● Clients who are aggressive or hostile may come into contact with you. ● Must be ready to address traumas that may trigger personal trauma and must work in conjunction with other collaborative staff members and communicate with many sources, including courts, the Department of Children and Family Services, LAHSA, law enforcement, and other community agencies. ● Must be able to work effectively and produce quality work under pressure.
