Participants are provided with a variety of services to promote housing stability and individual well-being. Housing Retention Case Manger is responsible for following up with Coordinated Entry referrals, for completing the lease packets, ensure the administrative compliance and collection of rent, delivery of rent, for site owned or managed by Westhab. DUTIES & RESPONSIBILITIES: • Responsible for conducting clients’ intakes and obtain needed documentation to ensure clients eligibility for the referred department/program. Maintains monthly and accurate and up today progress notes in HMIS as directed., • Able to travel within the Westchester County and attend DSS/SSI/SSD or medical appointment for the clients if needed., • Able to work one or two weekend day in the year as requested by Director, if needed. Able to work some evenings during the month if needed and as requested by Director, • Works closely with the Department of Social Services and the Westhab Housing Department staff to achieve client’s permanent housing goals as well as with shelter and transitional shelter staff., • Works closely with Westhab various department to ensure compliance, and program safety; Compliance Department, Security Department and the Facility Maintenance Department., • Attends weekly cross departments meetings and others as required. Responsible for keeping the client’s folders up today., • Requires the completion of various programmatic time sheets and activity log along with maintaining Google calendar updated., • Ensures that clients are following the program rules and regulations. Responsible for conducting daily wellness check with the most vulnerable clients. Serves as mediator for the clients when needed. Job Description Client Care Coordinator Supervisor PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. PRIMARY RESPONSIBILITIES: • Complete new resident Intake arrival packets, exits logs, daily occupancy forms., • Utilizing the Department of Homeless Services (DHS) Client Assistance and Rehousing Enterprise Systems (CARES) to complete Intake Check-in, Discharge Conditional Intake, • Review, Nightly Attendance Roaster Incident reports., • Receive food deliveries and confirm items received/receipts for accuracy by signing and dating form., • Report all inconsistencies with food deliveries to appropriate Shift Supervisor and Director of Program Operations., • Distribute meals to residents and dispose of expired food items as outlined by DHS’ Food Distribution Guidelines., • Monitor logs to ensure clients are signing the Daily Attendance Roster when entering and exiting the facility., • Provide clients access to rooms and assist in the facilitation and completion of room transfers/swaps., • Collect trash in office and dispose trash in designated areas., • Distribute DHS notices (i.e., 4002 notices, appointment slips, mail, etc.) to residents., • Adhere to DHS housekeeping mandates by cleaning office space, microwave, refrigerator, and bathroom at the end of every shift; always maintain a clean and neat office space., • Bag and tag client personal belongings where needed and report room cleaning and repair needs to Shift Supervisor., • Collaborate with on-site Shift Supervisor to ensure a safe environment for all staff and clients., • Conduct hourly rounds of the facility to ensure all room doors are secured, clients’ personal belongings are secured., • Conduct safety checks of clients staying in facility due to illness, mobility issues, etc., • Ensure clients are responsive during safety checks., • Immediately alert medical staff and immediate supervisor of clients found in distress., • Must be able to rotate shift at the discretion of the DPO to meet the needs of the program., • Conduct Operation unit inspections during the new Intake process., • Maintain all logs (Attendance, ACS, emergency contacts, sign in/ out logbook) and submit logs on the 1st of every month to Director of Operations., • On the1st of every month create a dead file of all the logs to be submitted for storage., • Adhere to the dress code policy for uniformed personnel and maintain a clean, neat, and professional appearance at all times., • Review with Shift Supervisor all paperwork and logs completed during previous shift before the end of tour., • Physically check all unoccupied rooms to determine status of readiness, repairs and cleanings., • Ensure all clean and ready units have clean linens and towels. The Staff on Duty teaches residents socially appropriate behavior by role-modeling. Job Description Position Summary: The Social Service Supervisor provides clinical and administrative oversight of the social service functions at a DHS-funded transitional housing program. Reporting directly to the Program Director, the Social Service Supervisor ensures the delivery of high-quality, trauma informed, client-centered services in accordance with H.O.G.A.R.’s mission, DHS/OTDA regulations, and contractual expectations.
