Job Description GA Family Services - Health Home Care Manager Reports To: Health & Family Services - Program Manager Job Type: Full-time, 40-hours per week. Although hours are generally scheduled for 1st shift, flexibility to meet family's and youth's needs is required. Salary Range: $22 to $30.40 per hour, commensurate with degree obtained and experience Please note: Although this position is based out of our Jamestown location, occasional travel to our Cheektowaga office may be required. Requisition ID: 1331Apply Purpose: The Health Home Care manager has the overall responsibility and accountability for coordinating all aspects of the member's care, ensuring compliance with delivery of the Core Health Home Services. The Care Manager is also responsible in documenting all core services in the electronic record. Qualifications: • Bachelor’s Degree in related field with relevant experience considered. Master’s degree in social work preferred, • Valid NYS Driver’s License required., • Conduct timely outreach, assess needs, determine eligibility, and complete Health Home enrollment and consent requirements., • Complete required assessments (CANS-NY) and develop individualized, family-driven care, safety, and crisis plans; update plans regularly based on progress or life changes., • Coordinate and integrate medical, behavioral health, social service, and community supports to meet each child/youth’s goals., • Collaborate with families, providers, schools, foster care/LDSS partners, and interdisciplinary teams to monitor progress and adjust services., • Support treatment adherence, appointment scheduling, and follow-up, including hospital/ER discharge planning and transitional care., • Respond to crises, provide on-call support as assigned, and ensure continuity of care across service settings., • Maintain required monthly client contacts based on acuity and document all services accurately and timely in the electronic health record., • Engages in regular supervision with own supervisor, including scheduled evaluations, • Actively participates in all in-service programs and scheduled meetings during your on-days, • Maintains ongoing and open communication with team members and promotes relationships of trust and respect with residents and staff, • Fulfills obligations inherent in the major systems of the GAFS and the LSS Human Resources Policies, • Demonstrates a commitment to organizational and department goals, • Shares positive and negative information directly with team members and confronts others; to utilize the organizational chain of command, • Practices good listening skills and communicates well both in writing and verbally, • Demonstrates personal and professional ethics, attitudes and value systems which enhance team functioning and to abide by the Standards of Confidentiality, • 401(k), • 401(k) Matching, • Generous Paid Time Off, Including 7 Holidays, • Health Insurance, • Dental Insurance, • Vision Insurance, • Life Insurance, • Short Term Disability Insurance, • Health Reimbursement and Savings Accounts (HRA/HSA) We can’t wait to hear from you!
